Answer: She broke the lock on the Headmaster's office

Before she committed suicide, Sakura broke the lock on the door of the headmaster's office, a clear violation of the rule stating that locks could not be broken or picked. She did this in order to aid her fellow students escape before she committed suicide. She would have been executed anyway for breaking the rule had she not killed herself before Monokuma got to her.
2. What was the "treasure" that Monokuma lost?

Answer: A key

Specifically, it was a key shaped like Monokuma that could open any locked door in the school. It is not clear when this key went missing but Monokuma was angry about it and implied that Kyoko was the one who stole it, which is later confirmed to be true.
3. One evening, Makoto fell sick and spent the day in bed. The next morning, he woke up to find almost all the other students in the gym studying what?

Answer: A broken Monokuma

During the night, Makoto saw someone in a mask hold a knife above his body. Soon after, Kyoko appeared and the masked figure departed. Makoto had been suffering from a high fever and was not sure if he was dreaming, having delusions or if this situation really happened.

Makoto woke up feeling refreshed and found the others, minus Kyoko, studying a broken Monokuma. The students believed this meant that Monokuma was now dead and they were safe from the mastermind. There was nothing of interest, however, inside Monokuma except what appeared to be a bomb which did not detonate.
4. Whose body was discovered in the garden?

Answer: Mukuro Ikusaba

Toko found the dead body in the garden when she went to retrieve an ax from the shed. This turned her into Genocide Jack and she went to fetch the others. The body was initially unknown as it was wearing a mask, the same mask Makoto saw in his room the night before. When they went to examine the body closer, it blew up leaving the top half completely indistinguishable.

However, the body has a tattoo of the wolf Fenrir on its hand. Fenrir is a militia group which Mukuro belonged to prior to entering Hope's Peak Academy. Makoto identified the body as Mukuro's when he found her student profile. With both Monokuma out of commission and Mukuro, whom was believed to be the mastermind, dead, the students think they are free to leave the school but stay behind to investigate the truth. However, Monokuma reappears shortly afterward.
5. Who were believed to be the only two possible suspects who could have killed the body in the garden?

Answer: Makoto and Kyoko

The body was not in the garden the night previous to its discovery as verified by both Byakuya and Hiro so it had to have been placed there during the night or early in the morning. Byakuya, Hiro, Hina and Toko spent the entire night investigating the damaged Monokuma and never left each other's sight. That only left Makoto who spent the night alone, sick in bed and Kyoko who vanished without explanation.

Initially, Makoto is the main suspect because the students believe the body is actually Kyoko's since she never showed up until just before the trial began.
6. Kyoko has scars on her hands.

Answer: True

Kyoko's hands are deformed from burns she sustained prior to coming to Hope's Peak Academy and she always wears gloves to cover her hands. During the class trial, the possibility of Kyoko being the masked figure who nearly attacked Makoto is brought up. Makoto says the person was not wearing gloves and hard perfectly unblemished hands. Thus, the person could not be Kyoko. Crucially, the person also did not have a tattoo on their hands, meaning it could not have been Mukuro either.
7. Which rule did Kyoko break?

Answer: She broke no rules

Kyoko did not break a single rule as she explored Hope's Peak Academy. She did steal the Monokuma key but there is no rule against stealing and she did not break any locks. She also accessed areas, such as the second floor dorms, which had not been opened to the students but these areas were not expressly forbidden to enter. Kyoko was always very careful to not break any rules which angered Monokuma because he could not punish her for her deeds.
8. At a point during the fifth trial, Makoto must make a tough decision between two choices, neither one of which will end pleasantly. What was this decision?

Answer: Whether or not to expose Kyoko's lie about the Monokuma key

When the class discussed how and if someone could have broken into Makoto's room to steal the knife, Kyoko mentioned she could not have done so because she didn't have a key to unlock the door. Makoto knew this assertion was not true because Kyoko had the Monokuma key which could unlock any door.

He knew that if he did not counter Kyoko's claim, Makoto would be the only other viable option to be the murderer. However, he also trusted Kyoko's word and struggles on whether or not to betray her.
9. In the bad ending, which character was executed?

Answer: Kyoko

Should Makoto decide to expose Kyoko's lie about the Monokuma key, the class will vote for Kyoko as the killer. Even though she is innocent, Monokuma will declare the class voted correctly and sentence Kyoko to be executed. Her punishment is the After School Lesson execution where Kyoko is tied to a school desk and sent on a moving conveyor belt. Monokuma will teach her - what appears to be - a sex education lesson while the belt slowly moves backwards under a compactor. Kyoko will be crushed to death.

In the aftermath, none of the five remaining students commit any more murders. However, they are stuck living in the school for many years. Hina has a child with both Makoto and Hiro while Byakuya also has a son with either Hina or (more likely) Toko who appears to have died a natural death. Should the player receive this ending, he or she will be taken back to the point in the game where Makoto must make his decision so the game can continue on.
10. Who saved Makoto from being executed?

Answer: Alter Ego

In the true ending, Makoto does not expose Kyoko's lie about the Monokuma key. Unable to convince anyone he is innocent, the class votes for Makoto as Mukuro's killer. Despite being innocent, Monokuma declares the class voted correctly and Makoto receives the same After School Lesson execution Kyoko received in the bad ending. Just seconds before Makoto would be crushed to death, Alter Ego opens a trap door in the conveyor belt, sending Makoto to safety in the trash dump.

It is later revealed in the next chapter that Alter Ego, before his execution in Chapter 4, hacked into the school's main system which is why he was able to stop Makoto's execution. Monokuma, however, erased Alter Ego permanently from the school.

It is never explicitly states but it is heavily implied that Monokuma intended for Kyoko to be executed here. As the daughter of the original headmaster, the execution theme fits her the best and Monokuma and the mastermind controlling him had been growing increasingly concerned about her antics and intelligence.
