1. "Fallout": According to the logs, who is the founder of the Brotherhood of Steel?

Answer: Roger Maxson

Roger Maxson founded the Brotherhood of Steel following his and his troops' desertion from the U.S. Army immediately before the bombs fell. The soldiers and their families moved to the Lost Hills bunker and established the Brotherhood, safeguarding pre-war technology from the wasteland.
2. "Fallout 2": What was the birth name of the Master?

Answer: Richard Moreau

The Master was born Richard Moreau in Vault City. Following his banishment he changed it to Richard Grey.
3. "Fallout": When did World War III begin?

Answer: October 23, 2077

The "Great War", as it was called, started October 23rd, 2077, and lasted for a grand total of around two hours. Every nuclear capable country launched their arsenal at each other and more energy was expended then every other war in history combined.
4. "Fallout": What was the project name the U.S. government used for the Vault Project?

Answer: Project Safehouse

"Project Safehouse" was commissioned by the U.S. not necessarily as a means of surviving the war, but as a massive social experiment to test people's behavior under harsh circumstances. However, there were a few "Control Vaults" that were designed to behave normally and eventually they created successful post-war settlements such as Vault City.
5. "Fallout": In what year was the Vault Dweller born?

Answer: None of these

This is actually a trick question since the game allows you pick any age from 17-35. The year of birth could be from 2126 to 2144.
6. "Fallout": The Forced Evolutionary Virus was originally called what?

Answer: Pan-Immunity Virion

The FEV was originally called the Pan-Immunity Virion and was being developed by West-Tek was a countermeasure to any biological weapons being created by China. It was renamed after the U.S. Military saw its potential as a super soldier serum.
7. "Fallout": What race is Harold?

Answer: FEV Mutant

While Harold may look like a ghoul, he is actually a mutant created by exposure to only a small portion of the FEV. This gives him a ghoul-like appearance without actually being exposed to severe radiation, making him one of a kind.
8. "Fallout 1 to 3": Who (or what) is responsible for starting the Great War?

Answer: None of these

While these are all touched upon as possible causes in the Fallout games, it is never actually said what the true cause was. Many believe China simply launched their nuclear arsenal and the other capable countries followed suit; An artificially intelligent Zax computer discusses the possibility that other A.I. launched the missiles simply because they were bored; On Mothership Zeta, a holodisk holds a recording of a military official releasing ICBM launch codes to the aliens.
9. "Fallout 1 to 3": How many known strains of FEV are there?

Answer: Four

There are four strains mentioned: The first is used to create the Master and his west coast super mutants, the second, a modified strain used to create the east coast mutants, the third, an airborne strain (FEV Curling-13) developed by the Enclave to eradicate all mutated humans on Earth, and a fourth, also created by the Enclave to contaminate Project Purity and destroy all mutated life in the Capital Wasteland.
10. "Fallout": According to Vree the scribe, why was the Unity doomed to failure from the start?

Answer: Super mutants are sterile

According to Vree's Autopsy, super mutants are completely incapable of procreating. This is due to the FEV seeing sperm and egg cells as "damaged" and subsequently destroying them. A holodisk of this autopsy and a successful speech check is the only way to convince the Master to give up peacefully.
