1. What is 'The Order of the Phoenix'?

Answer: a group of witches and wizards joined together to fight Voldemort

The Order of the Phoenix was originally formed during Voldemort's first rise to power, before he lost his powers trying to kill young Harry. When Voldemort returned to power, the Order reformed and began soliciting new members. Dumbledore is the leader of the Order, and most of Harry's adult friends are members.
2. A group of students form a secret study group to practice Defense Against the Dark Arts. What did Cho Chang suggest they call this group? (This is not necessarily what they end up calling the group.)

Answer: Defence Association

Cho was voted down and they called the group Dumbledore's Army instead. This name was chosen because Cornelius Fudge (Minister of Magic) seemed paranoid that Dumbledore was forging an army to use to push Fudge out of office. The students decided it was fitting to use the name to show their support of Dumbledore.

Answer: True

Under the leadership of Fudge, the Ministry of Magic has launched an attack on any member of the wizarding community who expresses the belief that Voldemort has returned, especially Dumbledore which is why the Order has to be secret. As for the DA, 'Educational Decree Number Twenty-four' made it against school rules for students to gather together in any kind of team or club.

Answer: he and Dudley are attacked by two Dementors

Because this was his second offense (the first offense was actually committed by Dobby), Harry was facing expulsion from Hogwarts, and was going to have his wand snapped in two by a Ministry official. However Harry was eventually acquitted of these charges when it was determined that he was acting to save his and Dudley's lives.
5. Who actually sent the Dementors to Harry's house?

Answer: Dolores Umbridge

It isn't until near the end of the book, when Professor Umbridge is questioning Harry in an attempt to find out where Dumbledore and Sirius are hiding, that she lets slip this little piece of information. She brags to Harry that while everyone was trying to find ways to discredit Harry, she was the one who actually acted and nearly succeeded in having Harry expelled from Hogwarts.
6. Only people who have seen death are able to see Thestrals. Who among the students were able to see them?

Answer: Harry, Neville and Luna

Thestrals are large, black, winged horses that Hagrid has bred a whole herd of, and uses to pull the school carriages. They are invisible to anyone who has not witnessed death. Harry has only been able to see them this year because of having seen Cedric die in the previous year.
7. Who accompanied Hagrid when he went in search of the giants?

They set off together at the end of 'The Goblet of Fire', but Hagrid reminds us of this when he's telling Harry, Hermione and Ron about what had happened when he'd been gone.

At first it looked like they were making progress, but the giant chief that they'd been negotiating with was killed, and the new chief had already begun negotiating with the Death Eaters. Hagrid did manage to bring his half brother home with him, so his trip wasn't a total waste of time.

Trelawney was the first to be sacked, and in a very public manner. Learning from her mistake, Umbridge attempted to oust Hagrid in the middle of the night, bringing five other wizards with her to help stun him. McGonagall ran to Hagrid's defense and was hit by four separate stunning spells, which resulted in her being hospitalized at St. Mungo's.
11. Did the stunning spells work on Hagrid?

Answer: No

Both Professor McGonagall and Fang were knocked out cold by stunning spells, but they bounced off Hagrid without any effect at all. He knocked out a few of his attackers, shouldered his unconscious dog, and ran off, presumably to join Dumbledore. Hermione reasoned that it was probably his giant blood that saved him, explaining that it was almost impossible to stun a giant.

It was easy to convince Fudge that Dumbledore had been plotting against him all along. Fudge and his cronies tried to bring Dumbledore in, but he proved far too powerful for them.

He also had to deal with his brother Percy turning traitor to the family and encouraging Ron to do the same, his father being bitten and nearly killed by a large snake, and his brothers (Fred and George) leaving Hogwarts to start up a joke shop.

Draco's father was sent to Azkaban because his role in the attack on Harry and friends at the Ministry of Magic finally revealed him to be a Death Eater. Draco blamed Harry for this so he, along with Crabbe and Goyle, tried to ambush Harry on the train home for the summer. Fortunately for Harry, members of the DA saw the attempted ambush and attacked first, and their combined spells hit Malfoy, Crabbe and Goyle, turning the three Slytherins into creatures resembling giant slugs.
