1. What do you call the first batter to come to bat in an inning in baseball?

Answer: The lead off hitter

Since he is starting (leading off) the inning, he is called the lead off hitter. That is true in every inning (and every half-inning, too).
2. In what position in the batting order does "the clean-up" hitter bat in baseball?

Answer: 4th

The "clean-up" hitter is usually the strongest hitter on the team. If he comes up with runners on base, his job is to clean off the bases by getting a hit that will score them.

Answer: 4

If you said four, you nailed it. He has to touch first, second and third base, plus home plate.

Answer: 360

Four times 90 = 360 (because there are four bases, including the home plate), so he has a bunch of running to do. But he doesn't have to run really fast. :-)
