We see this sign a few times throughout the movie. We see a close-up of it at the beginning, when Duane checks into the seedy place and at the end, when Belial and Duane hang out of the window and hang onto the sign itself.

Answer: Hamburgers, raw hot dogs

Shortly after the twins check into the hotel, Duane asks the man at the front desk if there is a place around to get some food and the man tells him there is a place nearby. Duane goes out and returns to the hotel room with a paper bag full of hamburgers. Duane helps himself to one, then pours the remaining hamburgers into the basket where we hear Belial devour them.

A little later, we see Duane go out and return to the room with a package of raw hot dogs, which he opens and dumps into Belial's basket.

Answer: Dr. Needleman's office

When Duane and Belial seek revenge on Dr. Needleman at his office, Duane meets Sharon at the front desk. They hit it off and make a date together. It is obvious this is Dr. Needleman's office because the doctor's nameplate says this and there is a close-up of the name on the door.

Answer: The typewriter

Sharon automatically assumes that Duane is there to fix the typewriter, because she is apparently expecting someone and Duane is carrying a basket. She tells him which typewriter is the one that needs looked at and proceeds to imitate the sound by making bizarre squeaking noises!

Answer: Like a squashed octopus

About halfway through the film, Duane runs into Casey at a bar and they get to drinking and talking. Casey finally asks him what is in the basket and he tells her the truth, most likely because he is very drunk. She thinks it's hilarious at first, but when he starts talking about their telepathic abilities and how Belial looks "like a squashed octopus", she starts to get a little freaked out.

Answer: Red

Casey gets ready for bed. She is wearing a smiley face nightshirt and she slips off her panties before getting into bed. After her freakout, we see Belial up close, holding and studying her panties. During this whole scenario, the panties are clearly bright red.

Answer: Their aunt

The flashback shows the brothers' birth and separation and the circumstances surrounding them. We can see their father blames them for the death of their mother and regards Belial as a monster he wants to rid the family of. After the separation surgery, Belial is removed and thrown out with the garbage. Duane rescues his brother and both of the boys plot and carry out their father's murder in a particularly glorious, campy gore scene. Their aunt takes care of the both of them, telling a social worker that they are TWO human beings when they are still attached and telling a story to both boys, holding Belial lovingly in her arms after the boys are separated.

Answer: She is on a dinner date.

Dr. Liftlander, Dr. Needleman, and Dr. Kutter all participated in the separation surgery of the adolescent twins. (Interesting side note: All of the doctor's names coincide with the tasks they performed. Dr. Needleman stuck the brothers with needles prior to surgery, Dr. Kutter cut Belial off of Duane with a scalpel,and Dr. Liftlander lifted the newly separated Belial off of his brother.) Dr. Liftlander is murdered by the boys at the very beginning of the movie. Dr. Needleman feels very uneasy after the boys' visit to him, so he calls to warn Dr. Kutter. Dr. Kutter seems very arrogant and unworried, pretty much telling Dr. Needleman that as far as she is concerned, the event never happened and to forget about it.

She also tells him that she would like to get back to her dinner and we can see that she is in the middle of a romantic dinner in her home with a hunky date.

Answer: A veterinarian

When the brothers go to Dr. Kutter's office to kill her, Duane sees her nameplate and says out loud, in a surprised tone, "She's a vet!" But this fact comes in handy and helps the twins easily enter her office because Duane tells her that he has a sick cat in the basket.

Answer: He kills Sharon.

Belial becomes enraged when Duane first kisses her and destroys their motel room. At the very end, Belial sits on top of a naked, sleeping Sharon and when she awakens, she screams and is murdered by him. Duane obviously becomes very upset with his brother and they fight, both of them ending up out of the window of their hotel, with Belial hanging onto the hotel sign with one hand and Duane's neck with the other.

At first sight, it may appear that the brothers die as well, but both of them make it into "Basket Case 2 and 3"!
