1. What is Henry's last name?

Answer: Letham

Henry Letham's last name is also an anagram of the name Hamlet, which is mentioned in the movie when Sam meets Athena at the acting class. Oddly, Athena is the woman whom Henry prepares to ask to marry before the incident.
In the beginning of the movie, there's a big car crash and a car ends up on fire. Henry sits in the middle of a bridge next to the car, gets up, and walks away. He later claims to have set the car on fire.
2. What kind of weather does Henry predict when he meets Sam in the psychiatry office?

Answer: Hail

In the beginning, Sam goes to work filling in for a psychiatrist at work, and he is told to take care of Henry Letham. Henry predicts hail although it's a nice day outside, and he says that he plans to see him over the next day. After all, he's planning to kill himself on Saturday at midnight, and he's been hearing voices in his head. Sam asks him to write down the voices but they aren't too significant.

The next day, Sam walks past some piano movers on his way to work and a little boy accidentally lets his balloon float into the air. Sam later sees this scene occur again in the same place on a different day.
3. What is the name of Sam's colleague who is cured of his blindness in the end of the movie?

Answer: Leon

Sam talks to Henry again at an art class to learn about his background, and Henry reveals that he killed his parents. As they walk through the building, many of the people around them are exact doubles, even triples of each other.
Later, Sam is with Leon playing chess. Leon is blind, and still manages to beat Sam because he has other things on his mind. One of these is the engagement ring he plays with. He plans to give this to his girlfriend, and ex-patient, Lila. Soon, Henry arrives and is shocked to see Leon. He claims Leon is his father, whom he killed years before.

In order to prevent Henry from hurting himself or anybody else, Sam asks another psychologist to help him by containing him for the weekend. They go to his apartment to find nothing but a small bed, a half-full case of bullets, and the words "Forgive me" written all over the walls and ceiling.

They check his answering machine and it's actually a call from Sam saying "Henry, hold on. It's going to be all right, just hold on." This is actually what Sam said to Henry while trying to save his life on the bridge.
5. What is the name of Henry's old dog?

Answer: Olive

In order to find more answers, Sam heads to Henry's old house and find Henry's mother (whom he claimed he killed). She takes him into the house, devoid of furniture, and offers him a drink. She leaves him with the dog, Olive, and heads to the kitchen, but he follows her and she disappears, soon, she starts appearing in different places around the house and claims that Sam is her son.

When her head starts bleeding, he offers to help, but Olive bites his arm. In the next scene, he's getting a rabies shot, and he is told that no one's lived in the house he visited for years.

Henry later tells Sam that Olive was put down when he was twelve years old.

Sam's second idea is based on a question he asked Henry after the art class. When Sam asked Henry if he had a girlfriend, he said he didn't although he liked a woman who served him coffee at a café on Canal Street. Sam searches every café and finally finds someone who knew Athena.

She directs Sam to an acting class where they talk and decide to leave. While heading down the winding staircase, Athena disappears and Sam slips and falls. He later wakes up and returns to the class to find Athena still acting.

In fact, she's saying the exact same lines she did when he entered the first time.
7. What is the name of Sam's girlfriend?

Answer: Lila

Lila was once Sam's patient, but they fell in love when she was feeling better. When she was at her lowest point, Lila attempted to slice her wrists in an attempt at suicide, but she lived and gained a brighter outlook on life. She is an aspiring artist and she lives in the same apartment as Sam. Their apartment reflects the Brooklyn Bridge as Henry sees it when he is dying on the road.

Sam heads to a local bookshop and finds a painting of the Brooklyn Bridge with Henry's name on the back. It turns out that Henry couldn't pay for books he wanted about a famous artist, so he traded the painting for them. This artist that he read about killed himself on the Brooklyn Bridge by shooting himself on his twenty-first birthday claiming that it was his greatest piece of artwork. Lila tells this to Sam and he rushes to the bridge to stop Henry.

When Henry visits his 'father', Leon, at his house, he magically cures him of his blindness and they make up despite the fact that he killed him. Leon then sets out and finds Sam near the Brooklyn Bridge. He claims that he always thought Sam had brown eyes and Sam talks to Lila on a pay phone to say that he loves her.

When he hangs up, Lila soon discovers that Sam was right about everything and she rushes to help him, but she's blocked in the apartment. Leon walks into the darkness as Sam rushes down the Brooklyn Bridge.

He catches up in time to talk to Henry, but Henry decides to make the ultimate choice and shoots himself in the mouth anyway.
10. What is Sam's occupation?

Answer: Doctor

Although it may appear that Sam is a psychologist throughout the film, he's not. In the end of the movie, Henry is on the bridge next to his crashed car, slowly dying. He hasn't been shot, but he's badly injured. Sam rushes over and helps him out saying that he's a doctor, and Lila helps out claiming to be a nurse. Soon, every familiar face from the movie shows up, and Henry drops the engagement ring from his hand. He pleads "Forgive me" and Sam says that it isn't his fault that the car crashed. Henry passes away on the bridge, while Sam and Lila decide that nothing can be done and go off together.

In the end, it's shown that the entire movie was an hallucination that Henry had while he lay on the bridge dying. Everyone involved, including Sam and Lila were around him as he died. Athena was his girlfriend, and she and his parents died in the crash. He planned to propose to her with the ring that night. His view, looking up at the bridge is seen throughout the movie in paintings, architecture, and even in Sam and Lila's apartment. The message on his answering machine was said by Sam while he was by his side, and his last words, "Forgive me", were seen throughout the movie. In the end, it's up to the viewer to decide whether the whole thing was a hallucination on his deathbed, or whether even that is just a hallucination.
