Mulder was surprised because he'd been reassigned by Skinner after the X-Files had been closed.

Mulder thought Skinner was alone in his office and went off on him from the doorway, complaining about the importance (or perceived lack thereof) of his assignments. Skinner then brought him into his office, revealing the meeting that was actually underway, and put him in his place.

3. Being checked out by a doctor, the sanitation worker said he'd be happy if she could give him something to get the taste out of his mouth. What did she give him?

Answer: gum

She handed him a stick of gum and said it would go away. I can't imagine what kind of taste would be hanging around your mouth after being dragged through sewer water.

He dropped a gate in the sewer, effectively cutting the fluke in half on its way back to open sea. It's not certain that the fluke died, though. In the last shot of the episode, the fluke's eyes opened, suggesting it was going to be able to regenerate and recover from the injury.
