Answer: it was hit by a truck

Betts was in the back with the patient, and had just saved his life, when a truck ran through the intersection and smashed into the ambulance. When Michelle crawled out and walked to the back to look for Betts, she found him on the sidewalk, decapitated.

Mulder told Scully that the cops had searched the entire hospital and had not found Betts' body. She told him she knew where it could be hiding and took him to the disposal unit, telling him, "All hospitals operate some form of medical waste processing.

This unit disposes of surgical remains - amputations, excised tumors. They're ground up and heated with microwaves and the result is a uh, sterile soot that's used as road fill." Scully put on a face shield and full arm gloves and opened the unit, ready to start going through its contents. Mulder was clearly grossed out, and told Scully that if she wasn't sure the body was in there, he didn't "see that there's any reason to disturb all this stuff." Scully ignored him and felt around a bit before telling him, "Mulder, I think I'm going to need your help. Your arms are longer." Mulder reluctantly put on the gear and reached into the waste, finding Betts' head.

Answer: eyes and mouth

Just as Scully was about to start slicing into the head, Betts' eyes popped open a bit. Scully jumped back from the examination table and, as she stood in awe, the eyes opened wider and the mouth opened up. Just as strangely, the eyes and mouth closed back and the head showed no further signs of life. Scully was effectively deterred from continuing on with the examination, though, until they could have the head chemically mummified.

Answer: Iodine

He found two large bottles of iodine and saw that the bathtub was full of iodine. If only he'd reached a little further into the bathtub, he would have found Betts' healing body lying inside!

Scully and Mulder took Betts' head to Chuck Burks' lab and had him do an aura photograph of the head. On the photo, there were faint shoulders emerging down from the neck.

She'd heard his voice on the dispatch radio and figured out where he was working. When she followed him to a secluded area of the parking lot, he hugged her, saying "It's okay. It's okay. I just wish you hadn't found me, that's all." She stood in his embrace and asked what he was talking about, but he stabbed her in the back with an injection needle, saying "I'm sorry" over and over.

He tore his thumb off, knowing he could grow a new one. When the security guard returned, all that was left was the bloody handcuffs and dismembered thumb.

Betts was riddled with cancer himself, and sought out cancer in others. He was very good as an EMT because he could just look at people and know when they had cancer. Mulder theorized that evolution had helped him develop the sense because he relied on cancer for survival.

When Betts needed to re-grow his thumb, he went to a bar and found a man with lung cancer from smoking to get what he needed.

Answer: by ambulance

Scully rode in the ambulance with Betts' mother as Mulder helped search the neighborhood for Betts. When they got to the hospital, Scully noticed blood dripping from the roof of the ambulance and climbed up to look on top. Betts ran around the ambulance and attacked her, throwing her into the ambulance.

As she lay on the floor of the vehicle, Betts approached her, saying "I'm sorry, but you've got something I need." He attacked her with a scalpel, but Scully fought him off, eventually shocking him with defibrillators, throwing him from the ambulance.

She woke up coughing and noticed blood on her pillow and dripping from her nose. This episode foreshadows the eventual discovery of Scully's cancer, most obviously in Betts' attack on Scully.
