The Endar Spire is the Republic capital ship you start the game on. It is attacked by the Sith and destroyed because the Sith want Bastila, a Jedi Padawan, alive. She has a power called Battle Meditation and it makes the battle being fought in favor of her side.

He escapes in a escape pod and shortly after you and Carth Onasi do also, but Trask Ulgo, your bunkmate, is killed by a Dark Jedi.

Answer: Taris

The whole planet of Taris is one big city. You start having these visions of Bastila on Revan's flagship after Carth brings you to an abandoned apartment.

If you poison the water, the Selkath officials ban you from entering Ahto City.

Elora is Sunry's wife, but Sunry was cheating on her with Elassa. If you go to the Republic Enclave after talking to the judges, you see a recording of Sunry killing Elassa.

If you go into the Mercenary Enclave, you will meet a Selkath named Shaelas. He wants you to investigate why so many Selkath, including his own daughter, are disappearing. If you go into the Sith base before you go to the Mercenary Enclave, a door is closed. You can go back in and that door will open after you go to the Enclave.
