She says her name to herself in the very beginning of the episode when she was about to tell The Monarch something very important but she was cut off when the Guild of Calamitous Intent barged in and captured both of them to be interrogated. It was never revealed about what she was going to say to him. Phantom Limb also called Dr. Girlfriend by her first name later in the episode.

This was brought up in one of Dr. Girlfriend's flashbacks. After Phantom Limb said a few phrases in French he then revealed to Dr. Girlfriend his true form, his limbs were invisible that gave him super strength. Phantom Limb said his limbs were invisible due to the fact that he was evil.

It was the other way around, The Monarch was one of Phantom Limb's henchman. He was referred to as Shadowman 9, The Monarch thought he was Shadowman 6. The Monarch never liked working for Phantom Limb and he wanted to leave the Guild to arch Dr. Venture. He created his own costume and soon became the butterfly crazed villain he was known as today after leaving Phantom Limb.

The Monarch planted a bomb in one of the halls but Dr. Venture wasn't in there at the time, Dr. Venture did get blamed for The Monarch's actions however. The Monarch was upset over two things. He failed to kill Dr. Venture and Dr. Venture took the "credit" for his own plan.

With the Cocoon destroyed and all of The Monarch's henchmen left stranded, Dr. Girlfriend's Murder Moppets took over as their new leaders. The henchmen did not like the fact that the Moppets took over and tried to fight them off, however they failed miserably. Henchman 21 and 24 decided to call Brock Samson hoping that he would kill the Murder Moppets for them.

Henchman 24, like The Monarch, was a henchman under Phantom Limb. He saw The Monarch getting into his own costume and warned him that Phantom Limb would kill him if he decided to leave the Guild. The Monarch told Henchman 24 that if he joined his group that he would be his number two. Henchman 24 did eventually join The Monarch however he remained number twenty four.

Answer: The Manotaur

The Monarch did not want to reveal his identity to Phantom Limb so he made up a name on the spot. Phantom Limb put The Manotaur on his revenge list. At the end of the episode, Phantom Limb found someone actually named The Manotaur and killed him thinking that he was the same person twenty years ago that stole his girlfriend.

His name was only revealed once in this episode. It was when Dr. Girlfriend was about to leave him for The Monarch. She said "I'm not coming back to the party, Hamilton. I'm not coming back ever! I'm gonna work with The..." She was cut off yet again when the leaders of the Guild of Calamitous Intent stepped in to see what was going on. Phantom Limb did not want to cause a scene as he was the host of the Guild Ball, so he just let The Monarch and Dr. Girlfriend leave.

He did not even try to fight the Murder Moppets instead he helped them rebuild the Cocoon. More villans showed up later to help rebuild the Cocoon. The henchmen did make peace with the Murder Moppets.

Even though Dr. Venture was seen in one of The Monarch's flashbacks and Brock Samson was seen helping the Murder Moppets, neither of these characters spoke in the entire episode. H.E.L.P.E.R. was heard in one of The Monarch's flashbacks when The Monarch was attacked by Dr. Venture's robot. Neither Hank nor Dean were seen, heard, or mentioned in any way in this episode.
