1. In 'Doctor Who: The Enemy Within' (the TV movie) was companion, Doctor Grace Holloway, killed?

Answer: yes

Yes, both she and Chang Lee were killed near the end of the movie, but both were resurrected not long after by the TARDIS.
2. Which of the following companions all died?

Answer: Adric, Chang Lee, Captain Jack

Adric was killed at the end of 'Earthshock' on a space ship that crashed into Earth, Chang Lee and Captain Jack were killed, but resurrected.

Katrina was killed in 'The Dalek's Master Plan', Jamie survived his travels with The Doctor, but lost his memories of them and Peri appeared to die at the end of 'The Trials of a Time Lord: Mindwarp', but is later revealed to be safe and well.

Jo, Polly and Dodo all survived their trip in the TARDIS, as did Ben, Steven and Mel.

Adam met the Doctor in the 2005 episode 'Dalek', but left the TARDIS at the end of the next episode 'The Long Game'. The Doctor met Peri in Lanzarote 1985 ('The Planet of Fire'), he met Jack in 1940s London ('The Empty Child') and Viki on the planet Dido in 2493 ('The Rescue').

The Doctor rescued highland Jamie from the battle of Culloden in 18th century Scotland in the 1966 episode, 'The Highlanders', at the end of which Polly asks if Jamie can join them, Jamie is very pleased at the suggestion, and the Doctor agrees as long as Jamie will teach him to play the bagpipes, though when Jamie finds a set of bagpipes in a trunk in the TARDIS later on, the Doctor doesn't seem too pleased.

He met the first Doctor in 'The Five Doctors', the second Doctor in 'The Web of Fear', 'The Invasion', 'The Three Doctors' and 'The Five Doctors'. He then became a regular during the third Doctor's era. He was a semi-regular during the fourth Doctor's time in the TARDIS and he met the fifth Doctor in 'The Five Doctors' and 'Mawdryn Undead'. Brigadier didn't appear again until the seventh Doctor's era, he met the sixth Doctor in the Children in Need special 'Doctor Who: Dimensions in Time'.

He met the seventh Doctor in 'Battlefield' and, though he didn't meet the eighth Doctor on screen, their paths have crossed in Big Finish audio adventures.

When Jo met The Doctor ('Terror of the Autons'), she is very quick to tell him that she took general science at A level. Liz was a physics expert, Peri's field was botany and though Ace was going to take science at A level, she didn't get to take her them before she was whisked away in a freak time storm.

Adam was kicked out of the TARDIS by The Doctor at the end of 'The Long Game' after he disobeyed the Doctor.

Tegan was on her way to Heathrow when she mistook the TARDIS for a real Police Box. The TARDIS took of with her still onboard, and then the Doctor spent the next series trying to get her back there. When he actually makes it, he and Nyssa leave Tegan behind, but she reappears in the next episode claiming to have lost her job.

Neither Leela nor Adric seemed to have had jobs, and Ace worked as a waitress on Iceworld.
