While it's clear that there's a disaster emerging in the world at the start of the film, somehow it's attributed to the famous, reclusive novelist Sutter Cane, a horror author known for his visceral, pulp horror novels that surpass even the works of Stephen King in sales. Cane is missing but the first few pages of his new manuscript, 'In the Mouth of Madness', seem to be causing a stir.

John Trent, an insurance investigator, discusses an upcoming case for Arcane Publishing with a colleague in a city diner when, in the middle of their conversation, a man covered in blood and wielding an axe breaks through the window next to their booth.

The man turns to Trent and asks him if he's read Sutter Cane before a pair of cops subdue and kill him. It takes a bit of investigating into the Arcane Publishing case to discover that the man was Sutter Cane's agent, and he'd read the first draft of Cane's newest book.

John Trent is asked to look into Cane's disappearance and to sink into his case he ends up purchasing all six of the author's horror novels, reading them and understanding his mind. While doing this, he discovers that a number of odd outlines on his book covers (designed by Cane himself) can be cut out to form the shape of the state of New Hampshire, and using this they can be used to locate the unknown town of Hobb's End.

During the night, Linda passes a biker several times and, before she knows it, the biker pedals directly into the front of their moving vehicle.

She and Trent get out to help what appears to be the elderly pedestrian, but he rides off into the darkness. They continue driving, emerging from a covered bridge into the daylight in Hobb's End, despite the fact that they'd been driving at night.

When Linda and Trent reach Hobb's End (to find a near-empty town), they head to the Pickman Hotel, the location from which Cane was said to be writing all of his books, to find Mrs. Pickman operating the establishment. She informs them that she's never heard of a man by the name Sutter Cane despite his fame as a writer and despite the fact that she's appeared in his writings as an elderly woman said to be an alien creature who's killed her husband in the hotel.

It turns out that this isn't far from the truth, but Trent doesn't find out until it's too late.

He and Linda check into their room despite Linda's growing fear of the small town.

As per one of Cane's novels, Linda and Trent discover that an imposing church can be seen from the window of their hotel room, and when they visit they find that something is seriously amiss as an unknown evil seems to be taking over people's minds (beginning with the children of the town). Trent writes off the church, but Linda returns to it alone at a later time to find Sutter inside.

He forces her to read his near-completed manuscript for "In the Mouth of Madness" and it drives her insane.

Trent soon realizes the horrible secret behind Hobb's End and tries to leave only to be foiled over and over again; every time he drives out of town, he winds up driving right back to where he started in the middle of the main street. He crashes his vehicle and wakes up in Cane's church to find that the manuscript is done and Cane informs him that he was created, in his final work, as a fictional character with a singular purpose-- to deliver the manuscript and spread the evil and end humanity. Cane then tears himself open, like the page of a book, to open the portal to the other side. Trent flees down a mysterious hallway (alone, since Linda can no longer go back) and emerges on a country road in the middle of the day.

It turns out that he's able to leave Hobb's End.

Answer: Linda never existed.

Trent ends up making it back to the city and, several times, he tries to get rid of the manuscript only to find it delivered right back into his possession. He ends up burning it in a motel room sink. Nonetheless, he returns to Arcane Publishing to find out that not only was he sent to Hobb's End alone (and that Linda never existed), but he delivered the manuscript months ago.

In fact, a movie adaptation of 'In the Mouth of Madness' is already in post-production, and whoever never read the book would likely see the film.

As people continue to purchase Cane's novel in bookstores, Trent ends up becoming deranged and, standing outside a bookstore to find a customer already starting to change, he pulls out an axe and commits a murder, not unlike Cane's original agent. He's placed into the asylum from the start of the film where, in the end, he ends up using a black crayon to mark crosses on himself and the padded walls to protect himself.

Unfortunately, it doesn't stop something horrible from occurring right outside his locked door, killing all of the workers and prisoners of the ward.
10. John Trent finally discovers that he's nothing more than a fictional character when he sees which of the following?

Answer: The film adaptation of 'In the Mouth of Madness'

Trent ends up leaving the asylum to find that the world has plunged into utter chaos. Anyone who hasn't read Sutter Cane's final work or seen the John Carpenter-directed adaptation of the novel is likely facing off against unfathomable creatures from beyond.

As he heads through his city, he comes across a movie theatre showing 'In the Mouth of Madness' and, sitting inside with a tub of popcorn, he watches himself (and Linda and Sutter Cane) on-screen. Of course he likely is mad, but it also confirms the suspicion that he's nothing more than a fictional character, doomed to be nothing more than a mechanism through which evil can spread and humanity can meet its demise.
