At the beginning of this episode, Kerry Weaver was seen entering a side room, ready to begin her shift. Having observed most of the ER staff enjoying a Valentine's Day celebration, she advised them to enjoy five more minutes before getting back to work. Following this, she checked on the X-rays of a patient, which led her to walk past one of the ER's side rooms. Noticing a bloody footprint outside the door, she entered the room, where she was met with a grisly scene. For on the floor lay two members of staff, unconscious and bleeding.

At the end of the previous episode, entitled "Be Still My Heart", medical student, Lucy had been on a Psychiatry rotation. During her shift, she was called down to the ER to consult on law student, Paul Sobriki. When Carter went looking for her in Sobriki's room at the end of this episode, he was stabbed in the back by the delusional patient, who appeared to be suffering a psychotic break. The last thing Carter saw as he lay on the floor, was Lucy's bloodstained body, lying just a few feet away from him.

Following Kerry Weaver's grisly discovery, the scene moved to a more jolly venue, to Mark and Elizabeth's attempts to sing karaoke. The pair were having drinks with Elizabeth's mother and Mark's father, who were sitting together, commenting on their children's lack of musical ability. However the light atmosphere did not last very long, as in the middle of their song, both doctors were paged back to the ER.

Following Weaver's discovery, the police were called to the ER. Fearing that Sobriki was still in the hospital, the police asked for a description, but as very few people had seen him, they had only his chart to go on.

While looking for an "infuser kit", Abby knocked down a sharp knife which had been hidden there. Smeared with blood, the knife was revealed to have been the same one the staff had intended to use to "cut the cake" at their Valentine's Day party.

When Sobriki's wife turned up at the hospital, having received a "message from Lucy Knight" asking her to come down to the hospital, it was left to Luka to explain the situation. Initially, Sobriki's wife refused to believe that her husband was responsible for what had happened to Lucy and Carter, insisting that "Paul couldn't hurt anyone". Maintaining that there must have "been some kind of mistake", she refused to accept the truth.

While Benton and Anspaugh were working on Carter, Anspaugh proved to be a calm and steady influence. For when Peter exclaimed that they had to get Carter's kidney out, Anspaugh forced him to remain calm and focused. As Peter panicked, Anspaugh ordered him to "calm down and listen", insisting that before they got to the point of no return, they should "check the source of the bleeding once more".

Romano and Corday worked on Lucy throughout this episode, first in order to stop her bleeding, and then again when she "threw a clot". However the second time they were unable to revive her, prompting Romano to throw a tray across the room.

When Chen and Malucci finished their shift, they went to Doc Magoo's for coffee, and found Abby and Luka sitting by the window. The four admitted that they did not want to go home, but felt that waiting in Doc Magoo's was better "than waiting upstairs". By the end of the episode, Haleh, Chuny and Lydia had joined them.

Romano said this to Lucy as he and Corday prepared to "numb the inguinal area and put a catheter in the femoral vein." Moments later however, Lucy was unconscious, and despite Romano and Corday's efforts, Lucy never regained consciousness.
