Abby said this to Susan at the beginning of this episode, after learning that almost everyone else was being evacuated. In response to Abby's question, Susan explained that she and Carter were unable to leave due to their "prolonged exposure" to the virus.

Susan tried to placate them by saying that those in charge had "promised a public-health nurse on duty around the clock", which prompted Carter to ask "What aren't they telling us?" While trying to evacuate a patient, Robert Romano backed into the rotor blades of a descending helicopter, losing his arm in the process. Falling to the ground, Luka and Susan rushed to his aid, and successfully managed to stabilize him.

Following the incident with Romano on the roof, Susan Lewis was left alone with her patient. When he stopped breathing she fought to save his life, and even managed to get him into the elevator in order to try and get some help. However despite her gallant efforts, help did not reach her in time, and the old man passed away.

Those who remained quarantined were extremely happy with Pratt when they found that he had managed to get some pizza smuggled in through the window of the hospital. A delighted Chen joked that she took back everything she had previously said about him. Meanwhile Carter revealed that the little boy suffering from monkeypox was "out of the woods", prompting the group to make a toast "to young Adam".

During this episode, Elizabeth Corday was shown having a hard time working in a hospital in London. Continually criticised for her "Americanisms" and attire, Elizabeth finally snapped when an English doctor remarked that America seemed to have confused her "in more ways than one." In a calm and controlled manner, Elizabeth responded with the above quotation, also adding that she had begun to realise that his disagreeable demeanour was simply his "general disposition".

When Carter was finally cleared to leave the hospital, he noticed that Susan Lewis was very tanned. When questioned about it, Susan revealed that she had been holidaying in Barbados. A guilty Lewis then added, "I had two weeks. What else was I supposed to do?"

Just before Abby left the hospital, she was seen asking Kerry to sign her timecard, in order to ensure that Abby was paid for "two straight weeks of overtime". A bemused Kerry looked blankly at the timecard, prompting Abby to respond with, "Do you think I'm kidding?"

After stepping outside, Carter commented that despite being stuck in the hospital for two weeks, he did not want to go home. Instead he suggested that he and Abby went to get some breakfast, but she refused. Moment later, the pair were seen walking on the beach, discussing chaos theory, before going for a swim.

When Carter and Abby were on the beach, Carter brought up the topic of chaos theory. Relating it to recent events, Carter mused that "A virus mutates in the Congo, we evacuate the ER in Chicago, Romano gets his arm cut off". Summing it up as "seemingly random events, all part of a larger equation", Carter commented that a butterfly flapping its wings in China, could cause "tornado halfway around the world".

This prompted Abby to ask, "So, what am I? The butterfly or the tornado?" Initially, Carter joked that she was "chaos in general", but when Abby stripped off and ran into the ocean, Carter exclaimed that she was "the tornado. Definitely the tornado."
