As her alarm goes off, Virginia wakes up and uses the restroom, saving her urine in a jar and applying it to her face as part of her morning ritual. Over breakfast, her step-daughter, Fay, urges her to stop her new skincare regiment; it's disgusting.

Their discussion escalates when Virginia pours herself a glass of asparagus and beetroot juice-- her obsession with anti-aging knows no bounds. While Fay prepares for a law school exam, Virginia watches her gardener out the window and considers how she can ask him out, knowing he's preparing for an upcoming divorce. Fay tells her that he buys a bottle of wine every Friday night from the same spot. Maybe she can make something happen.

Answer: They went to college together

Virginia ends up at the wine shop dressed in her nicest casual outfit and, unsurprisingly, runs into Bernie when he comes in. He comments that she looks positively wonderful and the two flirt a bit, but the organized run-in comes to a halt when Bernie's current fling, Cassie Brooks, interrupts the chat.

As it turns out, Cassie, who looks quite radiant, went to college with Virginia; they lived in the same dorm. As the couple finishes their shopping, Virginia has to ask Cassie how she keeps looking so young. Diet and exercise, Virginia claims, are the key, but Virginia calls out the lie.

She agrees to stay away from Bernie, and that's enough for Cassie. She hands over a card for Dr. Enid Perle, M.D. F.A.C.S., and tells her to call and book an appointment.

Answer: A worshiper

It takes little time before Virginia ends up at her consultation. There, she admits that she believes herself to be hideous. She never, in a million years, would have believed she would go to a plastic surgeon, but she begs to be taken as a patient so that she can cope with her life. Dr. Perle initially refuses to treat Virginia, but after hearing her out, she concedes to helping her.

She says she's a worshiper, and not a doctor; she revels in the beauty of the human body and seeks to prevent it from being weathered by time. If Virginia wishes to be her patient, she must consider herself worthy of worship. Virginia says that she doesn't feel that way about herself, but she wants to, and with that honest answer, Dr. Perle asks her to come into her operating room.

Dr. Perle insists, as Virginia takes a seat in the operating chair, that any notions she may have about reconstructive surgery be put aside as she won't be making cuts or slices into her to achieve their goal. She uses a proprietary augmentation and skin rejuvenation technique that she has perfected over years of travel and study; no one else does it. It's secretive, but there's a reason for it. When the time comes to show Virginia the cost, she balks a bit, but Dr. Perle reminds her, it's a service she won't get anywhere else, so it will be expensive.

Virginia speaks to her business manager and he attempts to dissuade her from taking the procedure. She's in too much debt. That night, while she applies a new face mask, she looks out her window to find Bernie and Cassie preparing for their night in the window next door.

It's why she signs the papers the next day.

As Dr. Perle takes Virginia's paperwork, she comments on her hands, saying that they are beautiful but it would be a shame if they didn't match the face she is expected to have after the procedure. Virginia, already worried due to the immense cost of the work, suggests it's not really in the cards, but Dr. Perle offers to complete the work on them for free, as a favour to her. If the vision isn't complete, she says, then it pains her.

That day, as Virginia is put under for her procedure, she notices that all of Dr. Perle's attending staff bear a butterfly tattoo. She can only see them briefly as they form a circle and begin to chant.

Answer: A week

Fay returns home to find that Virginia has come back from her procedure covered in thick bandages on her face and hands. Virginia begs her step-daughter for painkillers, having her feed her one from the bottle in her purse while she explains the reason behind her excruciating pain. The worst part is that she has a week of further recovery. Fay takes this opportunity to bring up the lack of money they have from the recent death in the family and it's during this that Virginia reminds her that they're only step-related. It's an emotional moment, and Fay departs to leave her step-mother to fend for herself.

That night, things only get worse. Virginia awakens only to find she's unable to grab her pills with her bandaged hands. In the darkness, she hears the sounds of inhuman creatures in her home and sees red-eyed apparitions charging at her. She's fortunate that Fay returns to find her on the floor.

Answer: A stay at a private retreat

When Virginia wakes up from her first night of agony, Fay serves her another pain pill and helps her up, claiming she came back because she doesn't know what Virginia's going through, but she shouldn't judge. Virginia never made her feel like just a step-daughter. She does have to ask, however-- what exactly is the procedure she got? They both realize that Virginia's bandages are very thick, but since Dr. Perle noted there would be swelling, it must be normal.

When Virginia is brought back to Dr. Perle's office for a check-up, she's reassured not only that the bandages will be removed in a day or two, but that the changes she's experiencing in her facial structure and the orientation of her bones is normal. Everything is going to plan. There is a problem, however, in that Fay seems to be a negative influence on her recovery. Dr. Perle recommends a retreat for the remainder of the recovery process; she has a place in the Santa Monica Mountains where she has many of her staff and patients on-hand for support, and the weekend there will coincide with the removal of her bandages. It's complimentary, she says, and it's enough to convince Virginia to go.

Before Virginia is whisked away to Dr. Perle's private retreat, Fay lets her know that she feels something is amiss. After checking the legal databases, she can't seem to find any trace of Dr. Perle or her operations, and it worries her. She has three malpractice suits; Dr. Perle is problematic. Virginia ignores her since she needs the recovery to go well.

Arriving at the retreat, Virginia is treated well enough but she realizes that she's the only person present wearing bandages. Dr. Perle performs one final house call and indicates that the bandages are ready to be removed. She does ask, however, that it be done at a little ceremony she plans to have later in the evening, with the other guests present. If Virginia is willing to let this happen, she will truly be among friends.

Fay, meanwhile, wanders at the retreat, having followed her step-mother into the mountains. As she walks through the woods nearby, she's caught, drugged, and dragged away.

Answer: A swine

Dr. Perle's ceremony is held and Virginia, the guest of honour, is brought out to the celebrating crowd. She's showered with compliments as she takes to a small stage and Dr. Perle says a few kind words before seating her patient. But before they can begin, the guards reveal Fay, who hid in the back of her mother's ride to the retreat. Virginia admits she had nothing to do with her step-daughter's decision as Fay is sent away, disappointed by her callousness.

Soon, however, the time comes to remove the bandages, and Dr. Perle has Virginia sit still while the procedure is completed. At long last, she's revealed to the crowd around her and she discovers that she has been deformed to look like a swine, complete with porcine facial features and hooves. Instead of joining their community as a member, she is there to join as a sacrifice to their god, Étaín, who depends on a sacrifice once a year on the day he slayed the ugliness in the form of a swine. As tradition, they must hunt her. If she can make it two miles to the Pacific Ocean, she can be free. She has a two-minute headstart.

Virginia races into the woods, quickly finding that the people of the retreat are in close pursuit with weapons in-hand. Unfortunately for her, they have good aim as well; they're quickly able to wound her before she crawls through a sewer tunnel. It's Bernie who finds her first, evidently a member of the community. She reaches to him for help, but it gives the group a chance to encircle her. As Cassie and Dr. Perle join the others, the latter takes a spear and makes the first strike, plunging it through Virginia before the others take their turns. When Virginia is killed, Fay is brought before her to say her goodbyes.

It's then that Dr. Perle reveals it was done for Fay as her birth mother was one of their community and a follower of Étaín. Dr. Perle tells her it's time for her to step out of her cocoon and spread her wings. She is one of the beautiful people.

When Fay returns to school, she does so with a butterfly tattoo on her arm. She meets a guy on campus as she heads to law school admissions and she finds that he shares the same tattoo, so they walk together.
