1. This episode opened with an extended shot of an American flag flying over an old western town. The scene was a flashback to hundreds of years ago, how many stars were on the flag?

Answer: 38

Flashing back to the old west like the show has done many times, viewers got another glimpse at the "old man" who heretofore hadn't been named.

To answer the question I didn't expect others to pause the show and count the flag, but 38 is the only viable option. There was never an American flag with 5 stars, and 13 stars would have been easy to spot. 52 isn't anywhere near correct as even in present day when the show aired there were only 50 stars on the flag. The stars correlate to the number of states in the U.S.
2. Who was left alive in the flashback scene to the old west bar?

Answer: The old man

The old man was asked if he loved Jesus Christ, at which point he shot the preacher in the bar. Then he told the man who had been singing to finish the song. During the rest of the song he shot everyone else in the bar. The last kill was the singer, who was killed via sword.

By the end of the episode the viewers realize that these haven't officially been flashbacks, but the seen playing out over and over in Hell.
3. Where was Jesse when he told the sheriff that he would see him Sunday?

Answer: In the back of a police cruiser.

Jesse had been arrested in the previous episode, so when he was first shown in this episode he was in the car.

Jesse and the sheriff had a conversation and then Jesse bailed out of the car and disappeared from sight.
4. Fiore and DeBlanc bought a trip to Hell from a travel agent.

Answer: True

The two walked into the travel agent and told her that they wanted to take a trip south. After a few options they spit out that they wanted to go to Hell. She booked the trip for Tuesday (as could be seen on the ticket that printed out) but first tried to get a better payment out of the deal. They threatened to turn her in instead of paying the higher price.
5. Who claimed to be dating the mayor when told they could have Jesse as a boyfriend?

Answer: Emily

The conversation happened in Tulip's house. Emily tried to tell Tulip that Jesse was in trouble and she responded that she was done with Jesse and Emily could have him as a boyfriend.

That's when Emily told her that she was seeing the mayor, which was at least partially true. She was sleeping with the mayor, but they hadn't been shown to be dating. She was actually hiding the sexual part of the relationship from her children.
6. Cassidy was fully healed from his sunlight exposure at the start of the episode.

Answer: False

Tulip had taken Cassidy in after he nearly died from the sun exposure. In the previous episode she was shown feeding him a dog. In this episode she asked Emily to watch him and feed him. There were numerous small animals, rats, gerbils, chickens, etc. to feed him.

When Emily peeked in the room, he was still a very disfigured mess.
7. Who did Cassidy feed on?

Answer: Miles Person

After realizing that the small animals weren't helping Cassidy to heal. Emily, who only in this episode found out that he was a vampire, called Miles over to "help" her.

Miles came to the house and searched for her, only to walk into the room with Cassidy. Emily appeared, slammed the door shut and waited outside as she heard Cassidy feed on Miles.
8. How did Jesse attempt to contact God?

Answer: Using Fiore and DeBlanc's phone.

Jesse and Cassidy made up and became friends again after Cassidy healed. The two of them decided to try to use the phone Fiore and DeBlanc had to call God. In order to do that they dug up the bodies of the two angels from the first time that Cassidy had seen met them. They had to use the hands of the two angels to make the phone work.
9. Where did Tulip go to "kill a man"?

Answer: Albuquerque

After a conversation with Emily, Tulip told her that she was going to Albuquerque to "kill a man". the next time she was seen on screen she was sitting in a chair listening to a voice mail from Jesse. Then she stood up, clutching a meat tenderizer and prepared to take her revenge on Carlos. He was the man she was seeking revenge on most of the season.
10. What was Odin doing in his office with more than a dozen of his workers?

Answer: Watching two of his employees fight.

Odin is always being shown doing strange things in these episodes. This week he was standing on his desk drinking booze and watching a street fight style brawl while a bunch of his employees circled around them.

Later he was talking with Donnie, who had made himself deaf in the previous episode.
