In the beginning of the episode, Lois asks the kids about their homework, and Chris claims that he's done his shoebox diarrhea about the Evolution of Man. When Lois corrects him by saying that the word is 'diorama', he realizes what he's done wrong. After this, Peter returns home with Lois' pregnancy test because she's nervous that she may be pregnant once again.

When Lois finds that her pregnancy test comes up negative, she and Peter decide to prevent any more future pregnancies or scares. Although Peter wants Lois' tubes tied, he and Lois watch a lengthy song and dance routine about vasectomies and Peter is somewhat convinced.

At the hospital, Peter accidentally knocks over some shelves containing sperm donations and a couple ends up using his donation.

When Brian takes Stewie to the park, Stewie gets bored pretty quickly, so he manages to take over the playground. In fact, for nine months, Stewie rules over the area, gets everything he wants, and gets the cutest girl in the park. He even gets men in black jackets and sunglasses to pass him Cheese Whiz. Soon, he goes to battle over the playground.

After nine months, Bertram appears and declares war on Stewie in order to claim the playground as his own. Eventually, the two have their armies set up as well as bases and aircrafts.
Bertram was first seen in season three in the episode "Emission Impossible" when Stewie went into Peter's body to prevent Lois from getting pregnant again.

When Stewie plans an air-based attack on Bertram's forces, he tells his army how to operate the airplanes. The duck button starts the plane, the cow button shoots the guns and missiles, and the clown button is just a nice extra button. When the armies go to battle, Bertram and Stewie blow up each other's planes, and when they both land in the trees below with their parachutes they decide to play 20 Questions.

After the air battle, Stewie gets a note from Bertram saying that he surrenders. He decides to kiss his girlfriend because of the victory and realizes she's wearing make-up to cover up her chicken pox. Stewie realizes this at the last minute and also contracts the virus.

He's forced to leave the battlefield but he vows revenge. Meanwhile, Lois starts gaining weight because she thinks that Peter no longer finds her attractive. When he takes her to a restaurant, he frequently hints about her recent overeating and he calls her fat, so when he warns the chef of her arrival, she vows to keep eating.

Stewie manages to bury himself in the sandbox and incapacitate all of Bertram's guards when he is free of chicken pox. When Stewie confronts Bertram, they fight all over the playground including on the monkey bars, slide, and see-saw (and they even have time to play on the slide). You may also notice that Stewie's (ex-)girlfriend decided to side with Bertram.

After the swordfight, Bertram admits defeat, and Stewie reclaims his rightful place on top of the monkey bars. Meanwhile, Peter continues to put down Lois because of her weight. When she finally gets fed up with it, they manage to rekindle the flame between them. In order to keep this up, Peter continues to feed Lois constantly.

Peter's plan to get Lois even larger is a success and she manages to get as large as Peter. One night though, Lois has a massive heart attack due to overactivity and has to be rushed to the hospital. The doctors are forced to give her liposuction in order to reach her heart, and luckily, she survives and returns to her old weight.

Joe appears with the guys at The Drunken Clam when Peter is considering the vasectomy. Shamus is also found in this episode at the hospital despite the fact that he hasn't appeared in an episode since "Perfect Castaway". Despite the fact that many supporting characters didn't appear in this episode, there were many occasions where famous people were used in jokes. Examples of these are Tony Danza, Sylvester Stallone, and Morgan Freeman.
