1. Where were we when we were helping the famous professor Mordin Solus cure a plague?

Answer: Omega

To recruit Mordin Solus, Shepard went to Omega where the professor ran a clinic. Mordin's clinic was in what became a quarantine zone because of a plague. Mordin had determined that the plague had been engineered, and had made an antidote. He couldn't get the antidote into the environmental systems, though, because he had too many patients to deal with, and the Blood Pack had taken control of the surrounding area.

He agreed to help Shepard in exchange for getting the cure to the environmental controls.

A Blood Pack vorcha revealed that the Collectors were the ones who had made the plague in the first place.
2. Where were we when we were recruiting the deadly assassin Thane Krios?

Answer: Illium

Nassana Dantius was a vicious business woman who often eliminated her rivals and anyone who might ruin her reputation. She would hire mercenaries and assassins to kill them, and bought impressive security. In "Mass Effect" Nassana tried to recruit Shepard to take out her own sister. Her sister was a slaver, and Nassana did not want her reputation to be ruined if someone found out about the connection. In "Mass Effect 2" she was so paranoid that she had her security mechs start killing construction workers in her own office tower.

Thane was a drell assassin who had given up on life. He had decided to make the universe a better place by taking out Nassana Dantius on Illium. He had been fully expecting to die on the mission. After the mission he was alive, but still ready to die, and so he accepted Shepard's request to help defeat the Collectors in what would probably be a suicide mission.
3. Where were we when we were helping Grunt through his Rite of Passage?

Answer: Tuchanka

Grunt was a young krogan, but tank grown. During his stay on the Normandy he started to get angry and violent. Shepard took Grunt to the krogan homeworld of Tuchanka to have him checked out by the local doctors. It turned out that Grunt had hit the krogan equivalent of puberty.

He was instructed to go on the Rite of Passage that every krogan adolescent did. After the Rite, he became a member of clan Urdnot. He was still violent, but was more focused and he gained a sense of belonging amongst his species.
4. Where were we when we were helping Samara the Justicar investigate the Eclipse sisters?

Answer: Illium

Samara was tracking an Ardat-Yakshi. Her investigation brought her to Illium, where her target had been aided by the local Eclipse Sisters. The Sisters had smuggled the Ardat-Yakshi offworld, and Samara was looking for the ship's name.

The Sisters were also under investigation by the police as part of a Red Sand smuggling operation and the murder of a volus.

Samara was causing headaches for the local police. She was a Justicar, and had a very binary view of what was right and wrong. Those who committed crimes in the eyes of the Justicar were often killed. While this was acceptable in asari space, if the Justicar were to kill a member of a different species it would have probably caused a massive inter-species ordeal.

Shepard offered to get the name of the ship for Samara in exchange for her help against the Collectors.
5. Where were we when we were saving the turian known as Archangel from a massive coordinated attack?

Answer: Omega

Archangel was a turian vigilante who worked on Omega. He caused havoc for the local gangs that ran the place, in an attempt to make Omega safer. It turned out that Archangel was actually Garrus, Sheparrd's turian squad member from "Mass Effect".

Garrus had angered the three largest gangs on Omega so much that they actually decided to cooperate to bring him down. They were in the middle of their final assault to kill Garrus when Shepard showed up.
6. Where were we when we were helping Thane stop an assassination?

Answer: The Citadel

While on the Normandy, Thane learned that his son had followed him in his career choice, becoming an assassin. Kolyat, though, hadn't received the training that Thane had, and was an amateur. Likely to get himself killed by the police or bodyguards of his target, Thane wnted to stop the assassination attempt.

Thane and Shepard went to the Citadel. First they talked to Captain Bailey, who told them about an informant named Mouse. Mouse was seen talking to a drell, and considering the rarity of drell, it was a good chance that the drell was Kolyat.

Mouse told them that Elias Kelham had recruited the assassin. Kelham was a human crime boss on the Citadel, and he bribed C-Sec officers to not bother him. At Shepard's request, Bailey brought him in for an interrogation. During the interrogation, Kelham revealed that he had hired the assassin to kill Joram Talid, a turian running for office. Talid did not like humans, and was rallying other aliens that did not like how quickly humanity had expanded into and influenced galactic politics. He even had his bodyguard shake down a human business.

Thane and Shepard followed Talid, hoping to find Kolyat before he could make the kill. Depending on choices made while following Talid, Kolyat could have pulled off the assassination and ran away, making Thane not loyal, or he could be stopped. If Shepard and Thane confronted Kolyat, Talid still had the chance of dying, except by Shepard's hand instead of Kolyat's.
7. Where were we when we were helping professor Solus rescue his former student?

Answer: Tuchanka

While on the Normandy, Mordin found out that his former student, Maelon, had been captured on Tuchanka. Considering how much the krogan hated the salarians, the krogan homeworld was probably not a safe place to be.

Shepard went on a rescue operation to the stronghold of clan Weyrloc, where Maelon was being held. When Mordin and Shepard found Maelon, they discovered that he had gone to Tuchanka willingly to help the krogan reverse the effects of the genophage.

At the end of mission, depending on the choices made, Maelon could have lived or died, and his work on the genophage could either have been saved or destroyed.
8. Where were we when we were flirting with an Ardaht-Yakshi?

Answer: Omega

Samara tracked her target, the Ardat-Yakshi, to Omega. When an Ardat-Yakshi bonded with someone, the connection would cause a brain hemorrhage in her partner.

The target's name was Morinth. There were only three Ardat-Yakshi known to exist, and Samara had three daughters. Morinth was one of them. Samara's other two daughters chose exile so they would not accidentally kill anyone, but Morinth had refused to live in solitude.

Morinth liked to seduce interesting people in the VIP lounge. Samara could not just walk in and kill Morinth. Morinth would have recognized her and slipped away again. Instead, Shepard was sent in to flirt with Morinth until she took Shepard back to her place. Once Morinth was out of the crowded areas, Samara struck. Both asari were powerful biotics, and locked in a stalemate. Shepard then had the option of helping Samara kill Morinth, or take Morinth back to the Normandy in place of Samara.
9. Where were we when we were helping Garrus track down Sidonis?

Answer: The Citadel

When you first met Garrus he was alone, being attacked by three separate gangs on Omega. The reason he was alone was because his entire team had been killed by a traitor. That traitor's name was Sidonis, and Garrus' sole focus after being rescued was tracking him down.

Garrus eventually learned that Sidonis was on the Citadel under an assumed name. In order to actually find Sidonis, Garrus had to talk to the man who had helped Sidonis disappear, a character name Fade. It turned out that Fade was the former C-Sec officer Harkin, and so he knew how to avoid the law. Once Shepard and Garrus fought through Harkin's security, Garrus got Harkin to set up a meeting with Sidonis.

Sidonis was a turian that felt awful about what he had done. He couldn't sleep, and food had no taste. Shepard had the option to either let Garrus kill Sidonis, or to convince him to let the other turian live in misery.
10. Where were we when we were helping Miranda protect her sister?

Answer: Illium

Miranda was a genetically engineered woman, created from her father's genes, and tweaked to be "perfect". When she was old enough, she ran away from her father's high expectations and harsh treatment of her. She ran to Cerberus, who hid her, trained her, and enlisted her into their ranks.

Miranda also had a much younger, but genetically identical sister, Oriana. Miranda took Oriana with her when she ran away, and Cerberus set her up with foster parents. Niket, Miranda's sole childhood friend and confidant, helped Miranda escape, but hadn't known about Oriana. When he learned about Oriana, he arranged for her to be reunited with her father. He reasoned that Miranda had been old enough to make her own decision, but that she had no right to make the decision for her sister.

Niket hired the Eclipse mercenaries to help transfer Oriana. He hadn't told Miranda's father anything, so only the Eclipse and Niket knew of Oriana's whereabouts. They all ended up dying, taking care of all loose ends.

After the mission, Miranda saw Oriana with her family. Shepard had the option to just leave, or to convince Miranda to talk to her sister.
