Valentina and her client get into a row over money, and the client attempts to assault her. Amanda, who is at the nightclub, sees what is happening and knocks out the client. Later that evening, Amanda kidnaps Valentina.

Valentina is given a box; inside the box is a Gigli wire saw and a machine that could suck out bone marrow.

To beat the game and survive, Valentina has to cut open her leg and suck out a certain amount of bone marrow in under three minutes. However, if the amount of bone marrow isn't enough in the time allocated, there is a second razor wire near her neck that would move towards her neck and remove her head. Valentina fails the game and is killed by the razor wire.

To survive the game, Mateo has to drill into his own head, remove tissue and bone from the skull, and put it all in a tank full of acid. Once the tissue and bone has been dissolved, it would make an electrical circuit complete to be able to get a key to remove a collar from around his neck. Mateo manages to remove the bone and tissue, but the amount is not enough for the acid to dissolve and make a circuit, and a timer stops.

A part of the two-piece collar then heats up and forms a mask over Mateo's face and kills him.

The reason Mateo is kidnapped is because he told John he was in safe hands with Cecilia and her team of experts, and so he becomes one of the targets.

Gabriela is raised up in the air and secured by a chain around her right ankle and left wrist. She is also subjected to ionizing radiation. She has to free herself by breaking the chains or bones with a sledgehammer to escape. Gabriela smashes the chain around her ankle so she can swing away from the ionizing radiation machine and then tries to remove the chain around her wrist.

However, just as the leg is free, the ionizing radiation machine re-positions. She breaks the chain around her wrist and falls to the floor.

As Gabriela survives the game but is badly hurt, John tells Amanda to call for an ambulance. Parker unexpectedly returns to the scene with a gun and makes John free Cecilia. Parker is really Cecilia's lover and partner in the scam, not a patient. John asks Cecilia to take Gabriela to the hospital, as she survived the game and that was within the rules of the jigsaw games.

However, Ceciliar refuses and steps on Gabriela's neck, breaking it.

Cecilia and Parker are put in a small room that would fill up with poisonous gas. As the gas enters the room, a small air vent opens up, just big enough for one person to get fresh air. Cecilia and Parker start to fight each other to get some fresh air. Parker starts to get the upper hand and finds a piece of metal and tries to stab Cecilia, but Cecilia fights back and bangs Parker's head against a control panel, grabs the piece of metal, and stabs Parker.

As Parker lays dying, his eyes start to dissolve from the gas, and eventually he dies from his stab wound and gas poisoning.

Although Cecilia survives the game, she dies from burns on her body and from the poisonous gas.
