Answer: Samara

In the original, Samara was played by Daveigh Chase, but she turned down the part for the sequel. Instead, Kelly Stables took the part. Chase's role as Samara is still used in the sequel (as archive footage from the first movie), but only as the "normal" looking Samara (as in not dead and creepy).
Naomi Watts still played Rachel and David Dorfman still played Aidan. Sissy Spacek played Evelyn only in the sequel; she wasn't replaced as she didn't exist the first movie.

Answer: He calls her "mommy" instead of "Rachel".

Whenever Aidan calls her mommy, she knows it isn't him because Aidan calls her only by her name. This is evident in both the first movie and the sequel. His eyes turn blue in the bathtub scene, but it is such a crazy and confusing moment that it is unclear even to Rachel what exactly is going on.

His tone of voice never changes; Aidan retains his body after all. It is evident that he has a pulse even when he's possessed by the hospital monitor that also displays his current temperature.

Answer: Simon Baker

All of the other choices were actors in the first movie. Martin Henderson played Noah, Brian Cox played Richard Morgan, and Adam Brody played Kellen (a minor role). Simon Baker plays the main male role in the sequel and he is killed by Samara in the end. Martin Henderson shared the same fate in the original.

It is odd that Rachel's is the only house on her entire block that lost electricity. It goes out when Rachel is talking on the phone, at the same time that Samara takes over Aidan again. She first goes to her work place with Aidan and then she goes to Max's house. That's where the bathtub scene with Aidan and Samara happens.

Answer: She puts prescription drugs in his food.

When Rachel decides to kill him, she knows he should be put to sleep first so Samara doesn't know what's going on. She tells him it's time for bed, to which he replies "I never sleep." She then makes him a peanut butter and jelly sandwich and crushes pills in it to make him fall asleep.

Answer: By sealing the top of the well

After Rachel regains control of herself in the well, she looks up top and notices that Samara's only way out is still open. So she climbs out of it and pushes the top of the well over the entire circumference just before Samara catches up to her and climbs out herself.

Answer: She follows Aidan's voice and jumps off the cliff.

A little skeptical at first, she digs up the courage to jump. After doing that, she awakens on the floor in her house with Aidan right next to her. Aidan calls her "mommy" here, and she tells him not to call her that for a while because that's what Samara used to do.

Answer: She thinks that they can't help him.

Max suggests that Aidan should go to a hospital, but Rachel refuses it. Later, after seeing Rachel try to drown him, he forces Aidan to go to the hospital without Rachel's consent. This leads to suspicions about Rachel harming her son, but nothing can be proved. Ultimately, Rachel is kicked out of the hospital and Aidan walks home himself, after having killed the doctor.

Answer: She has to go to sleep.

Rachel falls asleep in her room after seeing Max dead. While asleep, she dreams she is lying in her bed with Aidan, and this is where Aidan tells her that she needs to drown him. Aidan says that Samara always makes him sleep now that she's completely possessed him. The dream ends once a door shuts loudly.

Answer: She comes out of the of their television.

Right after Aidan regains his senses, he and Rachel hear the hum of the television. Samara climbs out of the well and steadily approaches the television, when Rachel decides she has to go in with her to take care of the problem. Samara's hands emerge from the screen and Rachel grabs her, and gets sucked into the television. The next thing she knows, she's at the bottom of the well.
