Answer: Dark Awakening

"Dark Awakening" was an episode of the original Generation One "Transformers" cartoon that takes place in Season 3 after the events of the 1986 movie. Optimus Prime is revived by the Quintessons as a zombie and was turned against the Autobots. He was programmed to lure the Autobots into a Quintesson trap. Optimus eventually realises that he's being used to destroy the Autobots and orders them all to retreat.

He flies his ship into the trap which is obliterated. Later Prime's body is shown to have been rescued by humans during the episode, "The Return of Optimus Prime".

Answer: He merges with Vector Sigma

"The Headmasters" was one of the Japanese exclusive "Transformers" series. It picked up after the events of Season 3 of the American series. Three episodes in, Optimus Prime once again sacrifices himself by merging with Vector Sigma. Vector Sigma, the mega computer on Cybertron was destabilizing after the Matrix energy was depleted in "The Return of Optimus Prime". Alpha Trion, Prime's creator met the same fate in the G1 episode, "The Key to Vector Sigma".

Answer: A Human

Optimus Prime dies in issue #24 of "The Transformers" comic book series by Marvel. A computer programmer named Ethan Zachary convinces Optimus Prime and Megatron to settle their battle in a video game. The loser of the game would be destroyed in real life. Megatron loses despite using a cheat code.

However, Optimus Prime forfeited as innocent civilians perished when Optimus killed Megatron in the game. Prime ordered himself to be destroyed and Ethan Zachary blew up the Autobot leader. Zachary later uses a disc from the game to help revive Optimus.

Answer: He disintegrates after getting hit by the Hydra Cannon

In the Armada series, the Hydra Cannon was a superweapon constructed by Megatron to harness the combined power of the three Mini-con weapons (the Star Saber, the Requiem Blaster and the Skyboom Shield). Megatron intended to destroy the Earth with the Hydra Cannon, but Optimus Prime places himself in the path of the weapon. Optimus shatters into white dust.

He is later revived by the Mini-cons.

Answer: Transform and Roll Out

"Transform and Roll Out" is the name of the pilot episode of "Transformers: Animated". Optimus Prime attempted to defend the city of Detroit against Starscream and was fatally wounded after the fight. He was revived by Sari Sumdac using her special key containing the healing power of the AllSpark.

Answer: Stabbed and shot through the back by Megatron

The Decepticons abduct Sam Witwicky from his college. Optimus Prime shows up to rescue Sam and attempts to fight Megatron, Starscream and Grindor all at one time. Prime kills Grindor and rips off Starscream's arm. However, Megatron sneaks up behind Prime and stabs him in the back and fires his cannon, blowing a hole in Prime. Prime is later revived by Sam using the Matrix.

Answer: The Swarm

"Transformers: Generation 2" was a short 12-issue mini series by Marvel Comics. In the final issue the Swarm, an entity formed from the creation of the second generation Transformers, threatened to destroy the entire Cybertronian race. Optimus Prime allows himself to be consumed by the Swarm. Prime's body is torn apart one molecule at a time. Once the Swarm reaches the Matrix it exposes the Swarm to its light and Prime is reborn into a new body.

Answer: Megatron shot him in the head

In the "Beast Wars" episode, "The Agenda", Megatron attempted to alter history by killing Optimus Prime before he awakens on Earth. He infiltrates the Ark and fires his cannon at Optimus Prime's head destroying it. Optimus Primal transfers G1 Prime's spark into his own until the Maximals can repair him, upgrading him into Optimal Optimus.

Answer: He transferred his mind to his Combat Deck

Optimus Prime controls three different components. His main body, the Roller drone and his trailer (aka Combat Deck). As seen in the Generation 1 episode, "Attack of The Autobots", Optimus can control all the parts simultaneously. In "Transformers: Escalation", Megatron tears through Prime's spark chamber and crushes it.

He then buries Roller under an avalanche. Prime manages to transfer his entire mind into his Combat Deck.

Answer: He plunges into Cybertron's core

While Optimus did have the Autobot base collapse on him, he was saved at the last minute by Smokescreen. He would later battle Megatron, who was possessed by Unicron. In an attempt to protect the AllSpark, Prime merges it with his own spark. He later sacrifices himself to return the AllSpark to Cybertron's core.
