1. "Arc of Infinity" sees the return of which Time Lord?

Answer: Omega

Omega first appears in the tenth anniversary serial "The Three Doctors", which is also the first time that multiple Doctors interact in the same story. Flavia doesn't appear until "The Five Doctors".
2. On Manussa, who tells Tegan that her whole business is a sham?

Answer: Fortune teller

At first, the fortune teller says that she sees the future, but then happily admits that it's all fake. She pretends to see things in a crystal ball but actually makes up everything to satisfy her paying customers. Then the Mara makes her crystal ball explode...
3. Why doesn't Ambril accept the Doctor's help or heed his warnings about the Mara?

Answer: Ambril thinks the story of the Mara's return is just a legend.

Ambril has heard many "warnings" of the Mara's return and puts no stock in them. He simply regards the Doctor as a lunatic who believes in legends. He even doubts that the Doctor is a doctor at all: "Doctor? Of what? I'm sure the man has no academic standing whatsoever." His predecessor, Dojjen, went into the desert to became a Snakedancer to prepare for the Mara's return; Dojjen and the Doctor defeat the Mara together by mentally resisting it.
4. "Mawdryn Undead" introduces Turlough, who is on a mission from the Black Guardian to kill the Doctor. How does he first try to do this?

Answer: hitting him with a rock

He gets right on it by trying to kill the Doctor at the end of part one. However, right as he has the rock over his head, the device the Doctor is fiddling with explodes, sending both of them backwards and knocking the rock out of Turlough's hands. "That shouldn't have happened," says the Doctor, but that accident spares him for the moment.
5. The Doctor shows himself willing to give up his regenerations to save Nyssa and Tegan in "Mawdryn Undead". Do either of them thank him for this?

Answer: Yes

After the Brigadier passes out from touching his older self, Tegan says, "Before we go, Doctor, thank you. You were prepared to risk everything for us." Ten stories later, the Doctor does indeed give up his life and regenerate to save a companion.
6. While sitting around in "Terminus", Turlough asks Tegan if she could ever do what?

Answer: Kill someone

Turlough is having second thoughts about killing the Doctor and brings up the subject with Tegan. She says she maybe could if she had to defend herself or save someone else. By the end of "Terminus", the Black Guardian is quite angry at Turlough for not killing the Doctor yet.
7. What does Striker say when he finds out that the Doctor is a Time Lord?

Answer: "Are there lords in such a small domain?"

Striker, like Marriner and Wrack, is an Eternal. As opposed to mortal Ephemerals (like humans), Eternals cannot die (they fade away and reappear somewhere else) and cannot come up with ideas on their own. Instead, they have to pull knowledge out of Ephemerals, which is also how they create their ships and everything in them.

In one scene, the Doctor has to throw an explosive off the ship, and even though Marriner could simply "think" it overboard, he doesn't because it literally can't occur to him to do that.
8. The Doctor wins the space race but turns down the prize of Enlightenment. Who is given a share instead?

Answer: Turlough

The Doctor says he's not ready for it and that no one can claim all Enlightenment. The White Guardian offers Turlough a piece of the diamond because both the Doctor and Turlough bring the ship into the harbor. By accepting it, Turlough would have to give part of it to the Black Guardian, which would also reaffirm the bargain to kill the Doctor. Instead, Turlough refuses it, which causes the Black Guardian to burn up and vanish, but not die. The Doctor surmises that Enlightenment isn't the diamond but the choice of whether or not to take it.

Marriner and Striker, like the rest of the Eternals, fade away and return to their void.
9. Which King of England knights the Doctor in "The King's Demons"?

Answer: John

Considering that he's not actually King John, but a robot facsimile called Kamelion, this knighthood probably isn't legitimate. However, the Tenth Doctor is knighted by the real Queen Victoria in "Tooth and Claw".
10. In "The Five Doctors", the first few Doctors are pulled out of time. The Fourth Doctor and which companion get stuck in the time vortex?

Answer: Romana

Producer John Nathan-Turner wanted all the previous Doctors to be in the special, but Tom Baker decided not to appear in it. The solution was to have him and Romana get stuck in a time eddy when unsuccessfully pulled out of time. Footage from the unfinished serial "Shada", which was meant to end season 17, is used in the scenes where the Doctor and Romana are taken out of time and returned.
11. Borusa gets his wish of immortality. What's the drawback of living forever?

Answer: He becomes part of a stone slab.

He joins all the others who sought eternal life: they become stone ornaments on Rassilon's tomb. Only their eyes can move. The Doctors decline Rassilon's offer of immortality after seeing Borusa's fate. Borusa first appears in "The Deadly Assassin" and later shows up in "The Invasion of Time" and "Arc of Infinity".

He is played by a different actor each time, so it looks like he's been burning through his regenerations.
12. In which story does the Doctor get into a sword fight?

Answer: The King's Demons

He is challenged by "the best swordsman in France" (who turns out to be the Master) and beats him. The Doctor also displays his fencing skills in "The Crusade", "The Sea Devils", "The Time Warrior", "The Masque of Mandragora", "The Androids of Tara" and "The Christmas Invasion".
13. Nyssa leaves the TARDIS team in which serial?

Answer: Terminus

She has a chance to use her science skills to help the sick people and Terminus, so she leaves the Doctor for a harder life. She kisses the Doctor goodbye and hugs Tegan before leaving.
14. What story is home to the infamous "No, not the mind probe" line?

Answer: The Five Doctors

The Castellan, played by Paul Jerricho, says it. Evidently he has good reason to fear it, because it kills him. Actors from one of the DVD commentaries pause and wait for the line to lampoon the actor's stilted delivery.

Answer: Enlightenment

In part three, while at Captain Wrack's party, he notices that his celery is wilting, so he takes it off and replaces it with a stalk from the table. Thus, he has swapped his Castrovalvan celery for Eternal celery. Both are mentally-made constructs, as the Eternals create everything on their ships from Ephemeral memories, and everything and everyone in Castrovalva was mentally made through Adric's thoughts.
