The fourteenth season of "ER" kicked off with a completely new face in charge of the ER. Following the resignation of Luka Kovač, a new ER chief took the reigns, in the form of new doctor, Kevin Moretti. This caused quite a stir in the ER and quite a few of the staff resented the changes made by Moretti. Greg Pratt was particularly annoyed that the Chief of Staff, Donald Anspaugh, had not considered him for the position.

In fact the position of ER chief was a role that Pratt strived for throughout the entire season.

At the beginning of this season Neela sustained serious crush injuries after being trampled on at an anti-war rally. Neela was eventually brought to the ER, where she was operated on by Lucien Dubenko, who saved her life.

After Gates lost his temper in the ER and attacked a patient, Moretti reassigned him temporarily to the ICU. Despite Gates' protestations, Moretti insisted that he needed to learn some control, and pointed out that the ICU was the best place for him to do so. Although initially skeptical, Gates excelled in the ICU, and eventually returned to the ER a much more controlled and humble man.

During this season, Abby's alcoholism resurfaced again, following an accident involving her son, Joe. With Luka in Croatia, Abby relied more and more on alcohol in order to get her through the day, even going so far as turning up drunk to work. During an ER night out, an inebriated Abby slept with Kevin Moretti, causing her to feel intense guilt. Even after admitting to Luka that she needed help with her alcoholism, Abby did not tell him about her one night stand with Moretti.

However after coming out of rehab, Abby did reveal the truth to a devastated Luka. Near the end of this season, Luka moved out of the home they shared as a family, because he could not deal with Abby's revelation.

Sadly for both Archie and Luka, their fathers died during the course of this season. However while Luka spent a great deal of this season in Croatia with his father, before he died, Archie did not get the chance to see his. HIV positive, ex-physician's assistant, Jeanie Boulet, made a special appearance in this season of "ER", in order to seek medical help for her HIV positive son, Carlos. In the years since leaving the ER, she revealed that she and her husband, Reggie had separated, and her first husband, Al, had died two years previously.

Initially, it was thought that Carlos was fine, but it was eventually revealed that he had a growth on his brain, prompting the doctors to fear that he had developed full blown AIDS.

Following Luka and Abby's marital problems, the former took a job working as a physician in a local hospice. Without discussing the matter with his wife, Luka took the job in an attempt to come to terms with the difficulties in his personal life. While working at this hospice, Luka developed a strong bond with one of the patients, an elderly man who taught him a great deal about forgiveness.

Midway through this season, Australian, Dr. Simon Brenner, joined the ER, as the new attending physician. Presented as a womaniser and an egotist, he angered Greg Pratt immediately by refusing to teach most interns, because he thought it was a waste of time.

However Pratt was left with egg on his face after complaining about Brenner to Donald Anspaugh, who turned out to be Brenner's uncle.

During the course of this season, Gates finally explained to Neela why he had nicknamed her "Mayday". During the pair's first meeting since Neela's accident, Tony revealed to her that the nickname "Mayday" came from his favourite Bond girl.
