Beecher, Keller, and O'Reilly worked together to manipulate Schillinger into having Lopresti give Andrew drugs, causing an overdose.

2. How did Ralph Gulino die?

Answer: Nikolai got Hoyt and the bikers to force drugs into him

Nikolai Stanoslafsky wanted Gulino's cell phone, so he payed Hoyt to have Gulino killed.

Answer: Supreme Allah

Keller and O'Reilly set Supreme up for murder, and Supreme knew exactly who had done it.
