Mac receives many anonymous calls from a person he labels the "333 Stalker" because all the calls occur at exactly 3:33 am. Mac also receives luggage with the numbers set back to "333". When he opens the luggage he discovers a bloody t-shirt of a young boy. Stella also receives gifts from the stalker, including a 3D puzzle of Chicago spattered in blood.

The clues lead Mac on a trip to Chicago to find a building connected with his past.

When the CSI team go to investigate the murder of a subway train operator, the train suddenly comes to life and starts heading for a collision. The CSIs try to think of ways to control or stop the hijacked train but nothing works. Danny, who had stepped off the subway to get his kit prior to the chaos, throws a rock and smashes the MP3 player causing the subway to run.

The train stops and the team notice a sign that reminds them of Mac's stalker.

The stalker sends Mac a puzzle of the Empire building and clues that all lead back to the building in Chicago. When Mac arrives, he finds a room with a skeleton hanging. Letters are written in blood and blanks are set up for a hangman's game. Taylor puts the letters together and discovers the missing word is "coward".

In the morgue, it is discovered the skeleton is a victim in his twenties who died thirty years prior from a gunshot wound.

When the Chicago Detective learns that Mac is connected to a murder and flashed his badge as an active police officer, she is infuriated. She calls Mac's supervisor in New York who is equally angered because he knew nothing about Mac's visit to Chicago beforehand.

They tell him he has twelve hours to return to New York City. The Chicago detective makes it clear that while Mac is in her city, he is a private citizen and it is not a joint investigation.

After spending time analyzing the crime scene in the Chicago lab, Mac meets up with an old friend, Jimmy. He reminds Jimmy of the night they saw his brother beat to death. He accuses Jimmy of being the stalker, making Jimmy very angry. Jimmy denies Mac's accusations and tells him to stay away. Mac finds out later Jimmy is innocent.

Flack meets up with Mac in Chicago and surprises him. During the visit, Mac fills Flack in on the details of his past.

Mac was sixteen years old when he went with Jimmy and his older brother to deliver newspapers. Sometimes Jimmy's brother would split the change he made with them if they tagged along. However something went wrong and instead of getting a payday, they watched a man beat Jimmy's brother to death. In the chaos one of the kids had handed Mac a gun and told him to shoot it. Mac was too afraid and froze. Jimmy grabbed the gun from Mac and shot the attacker. However the kid was beaten so badly he died later.

Stella discovers the real identity of Mac's stalker is Drew, a man she had been bumping into a lot during her work. Stella thought Drew had been sending her gifts and presents because he wanted to date her and is horrified to learn he was only trying to hurt Mac. Mac puts the pieces together and discovers that he knows Drew by another name and Drew is connected to the tragedy in his past.

Mac tells the police chief it is not about his badge, it is about him. He explains the only way to find out what is going on is to play the stalker's game and finish it. The chief considers it, and instead of pulling Mac off the case, surprises everyone by asking for one of their bullet proof vests. He joins Mac and the team in the raid of the stalker's apartment.

The stalker turns out to be Jimmy's younger brother, Andy, also known to the CSIs as Drew Bedford. To Mac's horror, he finds out Andy also witnessed his brother's murder. Andy holds it against Mac that he didn't have the guts to pull the trigger and Mac tries to remind him he was just a kid. Because Andy lost his brother, he attempts to destroy Mac's family, the CSI team.

He rigs a trap to shoot Mac or anyone that walks through the door. While they wait, Andy parades a newspaper article in front of Mac's face about how the city acknowledges him as a hero.

He tells Mac that the thing about heroes, is someone always rises up who knows their past and who they really are.

Mac's stalker booby traps where Mac is being held hostage to shoot Mac or anyone else that enters the door. However he doesn't count on his brother Jimmy trying to intervene. Jimmy wants to stop his brother because they have already lost one member of their family and he doesn't want to lose another. Mac's stalker is horrified when Jimmy enters the door and his own trap shoots him. Thankfully though, Jimmy os wearing a bullet proof vest and lives through the ordeal.

Mac handcuffs the stalker but does not kill him. Later when Flack asks him why, Mac replies that the two brothers have already lost enough.
