In "This Side of Paradise", mysterious spores had sustained the members of the colony on Omicron Ceti III, despite constant exposure to harmful radiation bombarding the planet. One colony member, Leila Kalomi (Jill Ireland), exposes Spock to those spores in an attempt to get him to return the unrequited love she had for him. For a while, the scheme works, but Spock (with Kirk's help) manages to shake the influence of the spores.
2. Shahna?

In "The Gamesters of Triskelion", Kirk, Chekov and Uhura are abducted from the Enterprise transporter pads to a faraway planet, where they are to be trained as "thralls" for gladiatorial combat. The thrall assigned to train Kirk is Shahna (Angelique Pettyjohn), but Kirk instead probes her for information and eventually seduces her.
3. Yeoman Martha Landon?
Answer: No
In "The Apple", the Enterprise landing party encounters a society that had literally stagnated because of the "god" they had worshipped, Vaal. Yeoman Landon (Celeste Yarnell) is Chekov's love interest in this episode.
4. Lenore Karidian?

In "The Conscience of the King", Kirk's friend, Tom Leighton, suspects a Shakespearean actor named Anton Karidian is really the infamous Kodos the Executioner. Later, Leighton is murdered, and the lives of Kirk and Lt. Riley are threatened. In an attempt to expose the elder Karidian, Kirk chums with his daughter, Lenore (Barbara Anderson).
In the end, Karidian admits he's Kodos and Lenore is revealed as the one who had killed Leighton and who tried to kill Kirk and Riley.
5. Lt. Marlena Moreau?

In "Mirror, Mirror", an ionic storm causes a transporter malfunction that sends Kirk & Co. to the Enterprise, but not the Enterprise they know. They find themselves in an alternate universe, one where deception and torture are the norm. But Kirk learns of a bonus: He discovers that his counterpart has a "Captain's Woman" named Marlena Moreau (Barbara Luna).
FAST FACT -- The shots of Kirk kissing Lt. Moreau were filmed at a later time, because Barbara Luna had strep throat during principal photography.
6. Rayna?

In "Requiem for Methuselah", Kirk, Spock and McCoy encounter a mysterious man named Flint. Living with Flint is a strikingly beautiful and intelligent young woman named Rayna (Louise Sorel). Almost immediately, Kirk falls for her, and Rayna begins to express feelings she had never experienced before. Meanwhile, Flint becomes increasingly jealous and the result ends up less than ideal; Rayna turns out to be an android incapable of expressing love and she dies as a result.
7. Eleen?
Answer: No
In "Friday's Child", Kirk, Spock and McCoy abduct the very-pregnant Eleen (Julie Newmar), who had been prepared to die after her husband is killed in battle. They run off to the hills and, thanks to McCoy's prior experience with Capellans and a little luck, they make it through all right. She has the baby (a boy), who is named for both Kirk and Bones: Leonard James Akaar.
8. Lt. Areel Shaw?
Answer: No
In "Court Martial", Kirk stands accused for negligence in the death of one of his crew, Lt. Cmdr. Ben Finney. At the lounge on Starbase 11, Kirk reunites with an old flame, Lt. Areel Shaw (Joan Marshall). While they do have a drink together, that is about all they do, because she's the prosecuting attorney in Kirk's case. By the way, Kirk is acquitted because Finney had faked his death.
NOTE: Though Kirk did kiss Lt. Shaw on the Enterprise bridge (!) at the end of the episode (at her request), there was no indication that they had pursued any further personal relationship together.
9. Edith Keeler?
Answer: Yes
In "The City On the Edge of Forever", McCoy accidentally overdoses on cordrazine and throws himself through The Guardian of Forever, resulting in the Federation becoming non-existent. Kirk and Spock go after him, finding themselves in 1930 New York City.
There, Kirk meets and falls in love with Edith Keeler (Joan Collins). Their relationship, however, turns out to be star-crossed. Kirk finds out that she must die in order for the timeline to be properly restored.
10. Dr. Janet Wallace?
Answer: No
In "The Deadly Years", Kirk and Co. are exposed to radiation poisoning, which causes all of the landing party (except Chekov) to age rapidly. Dr. Wallace (Sarah Marshall), another of Kirk's old flames, tries to rekindle their relationship, but the aging Kirk turns her down, saying "What are you offering me, Doctor, love or a going-away present?"
11. Dr. Miranda Jones?
Answer: No
In "Is There In Truth No Beauty?" the Enterprise welcomes aboard Medusan ambassador Kollos. With Kollos is one Dr. Miranda Jones (Diana Muldaur), Kollos' telepathic envoy, and instrument specialist Lawrence Marvick. While Kirk is among those charmed by Dr. Jones, their relationship remains purely professional.
Besides, she is so committed to her duty to Kollos that she had turned down Marvick's marriage proposal.
12. Dr. Ann Mulhall?
Answer: No
In "Return to Tomorrow", the Enterprise takes aboard the "essences" of Sargon, Thalassa and Henoch, members of a long-extinct race. They "borrow" the bodies of Kirk, Dr. Mulhall (Diana Muldaur, again), and Spock, respectively, to build androids so that they may again live corporeally. Sargon's love for Thalassa was beautiful and passionate (and a little sappy), and Kirk and Mulhall's bodies were used extensively to portray this.
But as themselves, Kirk and Dr. Mulhall feel uncomfortable being that close to each other (as the ending of the episode demonstrated).
13. Drusilla?
Answer: Yes
In "Bread and Circuses", Kirk & Co. discover an Earth-like planet whose society is a modern-day version of Ancient Rome. When they are captured, Kirk is interrogated and then sentenced to die in their gladiatorial "arena" (actually, it's a TV studio, complete with canned audience). Prior to the sentencing, Kirk is allowed one last night "as a man," with a slave girl named Drusilla (Lois Jewell).
14. Droxine?
Answer: No
In "The Cloud Minders", the Enterprise arrives at the planet Ardana to pick up a shipment of zienite. Complications ensue, and Kirk and Spock spend some time in the cloud-city of Stratos. There, they meet Droxine (Diana Ewing), daughter of Ardana's Ruling Council High Advisor, Plasus.
While Kirk spends much of his time trying to arrange the zienite shipment, Spock is the one who is entranced by Droxine!
15. Natira?
Answer: No
In "For the World Is Hollow and I Have Touched the Sky", McCoy tells Kirk he has a terminal illness and that he has one year left to live. Meanwhile, the Enterprise encounters the asteroid-ship Yonada. Once inside the "underground" city on Yonada, McCoy falls in love with their high priestess, Natira (Kate Woodville).
She returns his love and, despite McCoy's illness, they get married.
16. Lt. Karen Tracy?

In "Wolf In the Fold", Scotty is accused of multiple counts of murder on the planet Argelius II. It turns out that the murderer is a non-corporeal entity named Redjac, who apparently had started out as Jack the Ripper. Lt. Tracy (Virginia Aldridge) is one of the murder victims.
17. Sylvia?

In "Catspaw", the Enterprise crew meets up with two aliens, Korob and Sylvia (Antoinette Bower). By using their powers to "transmute" matter, they nearly succeed in driving the Enterprise away. But Sylvia is corrupt; she tries to get Kirk to join her (she wants to get rid of Korob) and he plays right into her hand, making her (initially) believe that she had him.
18. Lt. Marla McGivers?
Answer: No
In "Space Seed", the Enterprise finds the derelict vessel S.S. Botany Bay, with dozens of people on board in hibernation. Ship's historian Lt. McGivers (Madlyn Rhue) is on hand to see the first person awaken on the vessel: Khan Noonian Singh himself.
She is immediately drawn to him and jeopardizes her career by helping him take over the Enterprise. She is later relieved of her commission so she could accompany Khan on the planet Ceti Alpha V.
NOTE -- In "Star Trek II: The Wrath of Khan", we learn that several of Khan's people, including McGivers, had died after being exposed to the Ceti Eels on that planet shortly after their arrival.
19. Kelinda?
Answer: Yes
In "By Any Other Name", the Enterprise is commandeered by the Kelvans, who wish to take the ship on a 300-year journey to their home in the Andromeda galaxy. Kirk & Co. notice that since the Kelvans must remain in human form to move about the ship, they decide to expose the Kelvans to various forms of stimulation. Kirk's job was to expose Kelinda (Barbara Bouchet) to love and Rojan to jealousy and hatred. Needless to say, it works better than he had planned.
20. Miramanee?
Answer: Yes
In "The Paradise Syndrome", Kirk accidently triggers an ancient device that erases most of his memories. After search parties fail to find him, the Enterprise abandons the search in order to intercept an asteroid on a collision course with the planet. Meanwhile, Kirk (now "Kirok") is welcomed as a god to the Native Americans on the planet.
He then falls in love with the beautiful Miramanee (Sabrina Scharf) and marries her. They live an idyllic life for months, until Kirk is exposed as a false god and the two of them are stoned.
Unfortunately, she dies as a result of the injuries she had sustained.
