Answer: Martin

Jon Gries played Martin, a former hunter and current mental patient. The men were easily admitted when they were both extremely forthcoming to a doctor about the truth of their lives. They continued to be honest while inside the place. All three incorrect choices are characters from "Lost".
2. "Sam, Interrupted" - Which hospital worker was the actual monster?

Answer: Karla

Karla was a wraith, and would infect people to crank up their crazy. This effect would make the brains she drained even tastier. Dean was able to kill her as Sam suffered the hallucinations she'd caused. Lara Gilchrist played Karla. She played Holly in an earlier episode called "Scarecrow" in season one.
3. "Swap Meat" - What was the name of the 17 year old who body-swapped Sam?

Answer: Gary

Gary and his friends were trying to steal Dean, to sell him off to a demon. Gary loved being inside Sam's body. He went from a short, plain, geeky kid in a high pressure IQ bracket to a very tall, handsome, well muscled demon hunter. It took Dean a few hours to realize he had an impostor on his hands. Colton James played Gary. He's guest starred on "Dexter", "CSI" and "ER".

It was actually Gary's pal Trevor who summoned the demon, and his female friend Nora who was taken over. She killed Trevor and went after Dean, as Sam was tied to a chair in Trevor's basement. Dean and Gary chanted the demon out of Nora, and he and Sam took the kids home. Nora was played by Sarah Drew. She has also guest starred on "Mad Men", "Grey's Anatomy" and "Castle".

Julie McNiven returned as the angel Anna, freshly escaped from Heaven and on a mission to stop the devil. Castiel had been the one to turn her in, and did not trust her. He made a plan with the brothers to kill her before she could get to Sam. Her plan was better. She traveled back to before Sam was born to kill their parents before Sam's conception.

Answer: John Winchester

After a successful first battle with Anna, Mary led the group to a hideout. Anna recruited a younger looking Uriel to kill Sam and Dean. She told him that she would be his boss in the future and that he'd been killed by the brothers. He agreed to the task and attacked the hideout.

In the battle, Anna injured John and killed Sam. In his weakened state, John Winchester allowed Michael to use his body to save Mary. While housed in his John suit, Michael told Dean exactly why Dean would allow Michael to use him (Dean) as his vessel.

Then, he sent Sam, Dean and Castiel back to 2010. He also killed Anna and wiped Mary and John's memories of this event.
7. "My Bloody Valentine" - The Horseman known as Famine had caused a town to be filled with various forms of starvation. As a result, what was Castiel's particular over indulgence?

Answer: over eating

Being an angel, food was not something Cas had to worry about. But, apparently his vessel could still take in food. So, Cas was seen eating more and more, finally culminating in a grotesque raw meat scene in the diner. In the meantime, Dean fought no starvation and Sam was fiending for demon blood.
8. "My Bloody Valentine" - Was Sam able to exorcise the demons from Famine's body?

Answer: Yes

Famine needed souls to feed itself, and used the demons' essence when that was all he had. He told Sam that the powers of exorcism Sam possessed would not work on him directly. Sam agreed, but was able to suck the demon essences from the horseman's decrepit body.

The starvation thrall was lifted from the town instantly. Dean found out that his hunger did not exist because he was already dead inside.
9. "Dead Men Don't Wear Plaid" - Whose wife returned from the dead?

Answer: Bobby's

Sam and Dean traveled to Bobby's home town of Sioux Falls, South Dakota to investigate paranormal activity. Initially, Bobby tried to throw the men off the truth. It was not long before they found out that it was because Bobby's long dead wife Karen had returned from the dead, despite the fact that she'd been cremated. Bobby had been forced to kill her years ago after a demon had taken over her body.

He had to kill her again, when she turned into a savage zombie like creature. But, before he had to kill her she told him that she understood why he killed her the first time, thus hopefully relieving some of his pain.

Answer: Yes

It was Death's way of sending a message. He was warning that anyone, in this case Bobby, who helped the Winchesters would suffer.
