Answer: Natural Law

Chakotay suffers a fracture to one of his legs after he and Seven crash land on the surface of the Ventu homeworld. Although communicating with the aliens and trusting their intentions is tough going at first, the aliens eventually treat Chakotay's injury by fixing it up with a splint, after which he's seen walking around the Ventu village with a cane for extra support. Meanwhile, Paris is ordered to take a piloting course at the instruction of the aliens governing the space station overlooking the Ventu homeworld as a result of his piloting violations while onboard the Delta Flyer.

After being given encrypted orders to fly through the subspace barrier en route to rescuing the stranded Chakotay and Seven, Paris violates the government's flight codes once again, showing off some wild, fancy piloting maneuvers at the horror of his instructor seated behind him.
2. In the same episode, Tuvok is served some replicated cake without eating it while Kes sees a holographic image of someone outside of the holodeck.

In both the beginning of the episode (before Kes's unexpected encounter) and at the end of the episode (when Janeway and Tuvok are now both aware of what Kes is trying to do), Tuvok is in Janeway's ready room, where the captain surprises him with a plate of Jibalian fudge cake from the replicator for his birthday. Tuvok never eats the cake in either case, only blowing out the candles to humor the captain before he and Janeway exit the room together and return to the bridge. Meanwhile, upon Voyager's second encounter with the vengeful Kes, they've planned ahead and rigged engineering to where the warp core is taken offline, a holographic image of the younger Kes being programmed to convince the vengeful Kes to change her mind about sending Voyager back in time to allow the crew to fall victim to the organ-harvesting Vidiians.

In the beginning of the episode during the past timeline, when Voyager is still docked at Utopia Planitia, Janeway is being taken on a tour of Voyager by the admiral. Upon entering sickbay, the admiral activates the Doctor's program, allowing Janeway to see Starfleet's newest EMH.

After a few blunt remarks by the Doctor, the admiral promptly deletes the program before continuing on with his guided tour. Then later in the episode, when it's Janeway's turn to correct the polluted timeline by apprehending the vengeful Captain Braxton, she's trying her best to avoid contact with anybody else on the ship while Voyager is under attack by the Kazon.

At one instance, Janeway hides around a corner and out of sight when she sees herself giving orders to an unnamed female crewman in a yellow Starfleet uniform, the two ladies quickly walking to some unseen destination.

Following the battle with the Nasari ship, Harry goes to sickbay to get treated for his injury, then checks in on the injured B'Elanna who, after noticing the strange spots lining the side of his face (as a result of the supposed 'Taresian genes', which is revealed to be false later on in the episode), calls him 'Spot' as Harry begins to exit sickbay.

Then at the very end of the episode, Harry is recounting his experience with the Taresian women to both Neelix and Paris in the mess hall. Commenting, in so many words, that he's satisfied with having the plain old Harry Kim back on Voyager, Neelix gives the young ensign a robust pinch on the face before retiring to his kitchen.

Both events happen towards the end of the episode after the crew realizes Voyager is trapped inside the singularity. When it's decided that a shuttlecraft firing a dekyon beam into the singularity will collapse the hole that Voyager accidentally flew into, Paris volunteers to accompany the captain on the shuttle.

However, Janeway explains that someone else who's more experienced in the area of science related to the phenomenon would be the better choice, enlisting B'Elanna to join her instead of her conn officer.

Then following that away mission in the shuttle, B'Elanna has taken over the position of chief engineer, shouting orders to several crewmembers before she remembers to say 'please' so that she doesn't sound so threatening. All the while, Janeway and Chakotay are standing in the upper level of engineering, watching down on her and the rest of the engineering crew while remarking about the challenges that lie ahead as the Maquis and Starfleet personnel begin the gradual process of integration.

At the beginning of the episode, Tuvok is unconscious and lying on a bed in sickbay as a result of being attacked by the cloaked Ba'neth that had come aboard the Delta Flyer. The alien named Naroq, who has been eagerly investigating the Ba'neth and is in sickbay as well, tries asking Tuvok a question before the Doctor points out that nobody will be hearing from Voyager's chief of security, given his unconscious state, any time soon. Later in the episode, while Tuvok is in his mentally-degraded state, he's preparing desserts in the mess hall under Neelix's supervision when Janeway reminds him about the Ba'neth cloaking frequency. Unable to say exactly what it is out loud in words, Tuvok instead gets creative and decorates one of his cakes with the cloaking frequency design that will allow the Ba'neth to finally become visible.

When the Satan's robot begins advancing towards Seven, she replies, "I am Borg", opening the hatch to the robot's interior and disabling it by manipulating the controls inside. Meanwhile, after the 'nocturnal aliens' have drained all of Voyager's power and turned the ship completely dark, Chakotay is traversing one of the corridors when he hears some slight whimpering. Shining his SIMs beacon around, he illuminates a frightened Neelix huddled in a corner, having succumbed to his nihilophobia (or fear of nothingness).

Paris's defiant behavior, which began a few episodes earlier in "Meld", climaxes in "Lifesigns" where, in one scene, Paris has arrived late for bridge duty for the second time. A disappointed Chakotay has allowed another crewmember to take the conn station, and after a pitiful plea to the captain, Chakotay begins to escort Paris off the bridge.

But the disgruntled pilot retaliates by shoving the commander to the floor, prompting Janeway to order Tuvok to escort Paris to the brig. Earlier in the episode, the Doctor had gone to the holodeck to visit Paris in the Sandrine's program to get advice about love and dating, as the EMH had caught the romantic eye of the Vidiian doctor, Denara Pel.

At the end of the episode, Janeway enters the "Adventures Of Flotter" program on the holodeck where she tells Neelix about the time she starred in the children's holonovel and accidentally flooded the entire forest.

Paris and Tuvok, along with Harry, Seven and an emotionally detached B'Elanna, are all on the holodeck designing the Delta Flyer. Paris excitedly orders the computer to add spoilers to the new shuttle called 'dynametric tail fins', but Tuvok promptly orders the computer to remove those very same 'dynametric tail fins', explaining to Voyager's pilot that they're not constructing a 'hot rod'. Later in the episode, Chakotay confronts B'Elanna in her quarters about the holodeck programs she's been running.

The two head down to the holodeck together, but before B'Elanna can input her commands into the control panel, Chakotay uses a voice command to run a program that B'Elanna doesn't want him to see. He enters with B'Elanna clutched closely at his side, forcing B'Elanna to identify a dead person he turns over on the ground, before the commander furiously points out several more of the dead former Maquis by individual name.
