The clock is counting down the amount of fuel still left in the generators that are providing power to the CDC. When that fuel runs out, the building will self-destruct in order to destroy and contain all the viruses within the CDC so that none of them leaks out into the world.

The outer doors are locked, and the men are trying to bust through the windows when Carol asks Rick if perhaps the grenade she's been keeping in her purse might be helpful. Rick found the grenade in the tank, and she's been holding onto it. Rick uses the grenade to explode one of the windows, which allows the group to escape before the whole place detonates.

Jacqui chose to remain with Dr. Jenner when the CDC self-destructed rather than face a continued life in the zombie apocalypse. Although Andrea also wanted to remain behind, Dale manipulated her to force her out. The others escaped just as the CDC was exploding.
