In that episode, the device implanted in Garak's head to help him resist torture was malfunctioning as he had deliberately left it on. Garak told a convoluted story about how he came to be exiled on the station due to the actions of another Cardassian called Elim. Bashir finally managed to speak to the retired Head of the Obsidian Order to get a cure and on inquiring what happened to Elim, the former director told the truth.
