1. The First and Twelfth Doctors were both refusing to regenerate in the same place. Where were they?

Answer: Antarctica

For the First Doctor, 'Twice Upon a Time' took place as the 1966 serial 'The Tenth Planet' was coming to a close. Time was frozen for the other characters, though Ben and Polly (the first Doctor's companions) did make a short appearance. As 'Twice Upon a Time' was recorded over 50 years after 'The Tenth Planet', Ben, Polly, and the First Doctor were not played by their original actors.

The production team did a great job of finding actors who looked just like them - especially David Bradley as the First Doctor.

Answer: A WWI captain

A British WWI captain was in a ditch on a battlefield at Ypres. Opposite him, also holding a gun, was a German soldier. The captain was preparing to pull the trigger in self-defense when time froze for everyone except himself. He was transported to the South Pole in the wrong decade and appeared at the point where the two Doctors were arguing.

David Bradley played the First Doctor in 'Twice Upon a Time', but he also played Solomon the trader in the 2012 episode, 'Dinosaurs on a Spaceship'. Mark Gatiss played Professor Lazarus in 'The Lazarus Experiment' and Gantok in 'The Wedding of River Song'.

He has also written numerous episodes of the show. Peter Capaldi played the Twelfth Doctor from 2014 to 2017. Before that, he played Lobus Caecilius, a Roman merchant in 'The Fires of Pompeii'. He played the civil servant, John Frobisher, in a 'Torchwood' story, too.
4. Why did the Twelfth Doctor keep telling the First Doctor that he "can't say that"?

Answer: He was being sexist.

When 'Doctor Who' first started in 1963, characters often - though not always - conformed to racial or gender stereotypes. The men would often head out to explore dangerous places after telling the female characters to stay put for their safety, or to look after an injured party. 'Twice Upon a Time' acknowledged this side of the First Doctor's character by making jokes of the overtly sexist things he said.

Answer: Heather came for her.

Heather, who first met Bill in 'Pilot', became a kind of supernatural being. When Bill died, Heather turned her into one of her own species so that they could see the universe together. In 'Twice Upon a Time', Bill briefly explained what had happened to her, but for some reason, Heather wasn't there. The Doctor continued to question whether Bill was a duplicate or not.

Answer: They extracted something from people just before their death.

Testimony was an organisation from the future. They extracted people from their timeline at the moment of their death to save their memories. The "glass lady" was like a hologram, projecting the memories of someone who had died. The Doctor kept telling Bill that she was a duplicate.

She argued that a person's life is made of memories; all of Bill's memories were inside her, so she was Bill.

Answer: Soldiers started singing 'Silent Night'.

The Captain in 'Twice Upon a Time' turned out to be a relative of the Brigadier and Kate Stewart. He was transported back to the battlefield ditch where he was about to die. Suddenly, someone started singing a Christmas Carol. The Captain was saved by the Christmas Truce of 1914.

Answer: Clara and Nardole

Testimony was able to recreate people from their memories. They were also able to restore the Doctor's memories of Clara after he lost them in 'Hell Bent'. The memory of Clara appeared to say goodbye to the Doctor, and so did Nardole. Bill and Nardole tried to convince the Doctor to regenerate, but the Doctor was tired of always losing his friends.

In the end, the Twelfth Doctor decided to give one more lifetime a go.
The Thirteenth Doctor's first words were "Oh, brilliant" before the TARDIS tipped up mid-flight and she was thrown out of the door. Her first full-length episode as the Doctor was 'The Woman Who Fell to Earth' at the start of series eleven.
