1. The fourth episode of "Ultimate Doom" is called what?

Answer: Thy Flesh Consumed

"Knee Deep In The Dead" is Episode 1, which was also available in the Shareware version of the original "Doom". "The Shores Of Hell" and "Inferno" were Episodes 2 and 3 respectively which were only available with the full version. "Thy Flesh Consumed" appeared in the later version which was upgraded to "Ultimate Doom".
2. Sever The Wicked (E4M3) from "Ultimate Doom" contains the most secret sectors of any "Classic Doom" level. How many does it contain?

Answer: 22

Most of the secret sectors in this level are contained in one area behind the North door in the octagonal shaped room with candles in four corners. Unfortunately only 90% secret is possible in normal play due to two of the four corners being flagged as secret.
3. In Ultra Violence mode, which monster makes the least amount of appearances on "Doom II"?

Answer: The Icon of Sin (aka The Demon Spitter)

There is only one Icon of Sin in the level of the same name, there are four Commander Keen's in the form of a hanging effigy in the level 'Grosse'. The Cyberdemon and the Spider-Mastermind make five appearances each; The Cyberdemon appears once each in 'Tricks and Traps', 'Refueling Base', 'Gotcha!', 'The Living End' and 'Grosse'.

The Spider-Mastermind appears once each in 'The Crusher', 'Gotcha!', 'Barrels O'Fun' and twice in 'The Spirit World'.

'Dead Simple' only has one secret, which is the rising stair which surrounds the exit platform and thus is impossible to avoid in normal play. There is a deathmatch exit which can be reached by using the walk through walls cheat and this will give you 0%. Additionally there is a tool assist which can be used to jump over the secret.

Answer: Backpack

Weapons and Ammo do not count towards your items score, neither surprisingly does Green and Blue Armor Suits and the Radiation Suit. Apart from those listed above the things that do contribute to the items score are; Computer Area Map, Health Bottles, Berserker Pack, Soul Sphere, Invulnerability Sphere and Invisibility Sphere.

Answer: 6

Nearly all of this level takes place in the eponymous tower, but because of the 2D nature of the "Doom" engine, all of the different levels have to be separated on the game map. The red and yellow key are found on the fourth floor and the blue key on the tower's roof

Makes its debut in the "Doom II" level 'O' Of Destruction'. Because of it's ability to resurrect the majority of demons from the dead, it is recommended to kill an Arch Vile first before any demons around it. The other demons an Arch Vile cannot resurrect are; The Lost Soul, the Pain Elemental, the Cyberdemon and the Spider-Mastermind.

Answer: 'Cyberden' and 'Go 2 It'

These two levels are most notable for the number of Cyberdemons that appear. A total of 17, regardless of skill level! 'Pharaoh' and 'Caribbean' are the Secret levels on "Final Doom: TNT Evilution", 'Warrens' and 'Fear' are the secret levels on Episode 3 and Episode 4 respectively of "Ultimate Doom", and 'Wolfenstein' and 'Grosse' are the famous secret levels from "Doom II".
