On a rainy night at Pinecrest Memorial Hospital, the lights nearly go out. A security guard lets a vehicle pass through the gates before lighting a cigarette and picking up his book. He turns around just in time to see the shadow pass behind him-- a patient from the hospital who recedes into the shadows when he calls in to have someone retrieve her. By the time he goes out to look for her, she's already gone.

Inside, a nurse, Claire Michaels, informs a woman that her son has a tumour on his thoracic spine. The tumour appears to be growing, but the mother doesn't have insurance. It's fortunate for her, however, that Dr. Nostrum, the chief oncologist, has determined that her son is eligible for the Springhill Program's endowment; they'll pay for all his treatment.

Nurse Claire starts to admit that working at Pinecrest is turning her into a soulless monster, and the fact that she has to turn patients away is the opposite of what she signed up to do weighs on her. She asks about the Springhill Program but gets brushed off since it's openly known Dr. Nostrum made the fund when his father died of cancer. The discussion comes to a quick end when the nurses turn the corner, right as the lights start to flicker, to find a patient standing at the end of the hall, mouth wide open, with a deformed, curled hand.

Claire and Lilly take the patient to one of the empty beds and find that despite the terrifying expression on her face, her vitals seem normal. She appears unable to respond, but she seems to understand that they're talking. Lilly reasons that, perhaps, their new patient is a victim of a new street drug. She asks Claire not to leave her alone with the patient; she makes her nervous.

As Claire takes the patient's blood, the lights begin to flicker once again, prompting Lilly to note her concern not only for the hospital as it seems to fall apart, but for the patient. As Lilly departs, Claire realizes that the patient may be trying to communicate with her, though her frozen mouth makes it impossible for her to understand a word. As an orderly comes into the room to change the bedding, she realizes that she missed a vial to hand off to Lilly. It means taking it down to the lab. Neither of them notice, in the meantime, that the patient is eyeing a pair of scissors on a nearby tray.

Claire drops the sample off in the lab and speaks to the technician there, but discovers that Lilly never brought the other vials by. She doesn't have time to question why before a scream rings from the other room. It appears that the patient got their hands on the scissors; the orderly bleeds out on the floor with the blades in his jugular and the patient is gone. All that remains is an X marked in blood on the wall.

After the attack, Claire ends up speaking to Dr. Nostrum, who attempts to comfort her by getting a security guard, Malcolm, to grab her a black tea. The doctor explains that they've combed the hospital-- twice!-- but can't find the missing patient. Nonetheless, Claire can't shake what she heard the patient trying to say to her, something like "Ward X". Dr. Nostrum jokes that it sounds like a pesticide while Claire looks down the hall and sees others glancing nervously at her. Malcolm is volunteered to give Claire a ride home on Dr. Nostrum's recommendation; she isn't needed for the rest of the day.

As Claire goes to get her things and wash up, she's found by the patient who, using a scalpel, removes her wristband and brandishes the tool. Claire washes the identification to read it, discovering her name is Alice Taylor, but there's no further clue as to what happened to her. By the time Claire turns back around, the patient has vanished.

Grabbing a flashlight and heading into the archival walls of files, Claire and Lilly manage to find Alice's file in short time and determine that she was admitted for skin cancer. More concerning are the facts that her attendant orderly at the time was Justin, giving the possible motive as to why she killed him, and that she allegedly died on the operating table-- something that should never have been possible for a skin cancer procedure.

As the lights begin to flicker, Claire and Lily realize that there's a patient transfer form in the file that intended to send Alice to Ward X. The problem is that Lilly's name is on the paperwork as the admitting nurse indicating that she knew the whole time that something was amiss. Lilly begs not to talk about it as she hyperventilates and she rushes to the roof for a cigarette with Claire in tow. They can't talk about it inside; Lilly doesn't want to talk about it at all.

On the roof, Lilly admits that she knew about Ward X but that she only helped, originally, with the effort to get people feeling better, and she never knew it would be so dangerous until the events of that evening. Every patient that went in, she noted, never came out, and if she ever said anything, she would be admitted herself. She worked hard to hide everything, but maybe, she says, she deserves what she gets.

Lilly doesn't admit to what she did with Alice's blood samples. Instead, she takes Alice's file, recommends that Claire leave and not come back, and throws herself off the roof of the building.

When Claire and Malcolm come down to the crime scene later on, Claire asks if he saw the file Lilly was holding. He did, he explains, but he saw an orderly come by and pick it up. He once again offers to take her home, but after he speaks to a detective with his statement. Dr. Nostrum, meanwhile, asks Claire to come to his office.

Claire takes a seat in Dr. Nostrum's office and explains everything-- she and Lilly went looking for answers instead of going home, and all of the files pointed to Ward X. Dr. Nostrum is upfront about this; it's the Springhill Program, and Ward X is where he executes it. The program was created with the aim to take out cancer, and as he alleges, it's on the verge of a major breakthrough of removing all the pain that comes with the disease. Claire argues that it's clearly causing suffering, and Dr. Nostrum agrees, but the ends justify the means.

Moving to a box on the bureau, Dr. Nostrum collects a syringe and attempts to sedate Claire, but she fights back, jamming a thumb in his eye and knocking him out. She grabs his ID card and makes to leave, taking a detour to her earlier patient's operating room in an effort to prevent him from suffering the same fate as Alice.

Meanwhile, outside, Malcolm is knocked out by two orderlies and carried inside.

Claire attempts to flee from security and, in doing so, she finds herself in a stairwell with Alice who, stumbling through the corridors, leads her into the service tunnels of Pinecrest all the way to a door simply marked with an X. Claire uses Dr. Nostrum's card and lets herself in.

What she finds is nothing sort of terrifying-- a hallway of cages, all containing despondent patients under various states of duress. At the end of the hall she finds a dirty laboratory and proof that Alice escaped through the ducts. The discovery is short-lived when Claire finds Malcolm in one of the cages. Unfortunately, this distracts Claire long enough for one of the guards to sedate her.

She only awakens to the sound of Dr. Nostrum's voice as he starts his procedure on Malcolm.
10. Who doesn't survive in the end?

Answer: Dr. Nostrum

Dr. Nostrum injects Malcolm with a solution that knocks him out and starts to take over his liver while he explains to Claire that an unexpected appendectomy would leave her with a cover for her untimely death. While he describes this, however, Alice emerges from the ducts, grabs a scalpel, and kills his nurse, giving Claire the chance to free herself and kill the evil doctor before she passes out.

When Claire awakens she finds herself in a hospital room upstairs under the observation of a detective and nurse who explain everything. Claire was found tied to the gurney in the basement while Nostrum and the nurse were found, dead, next to her. Malcolm was sent for further tests while Alice, wandering the halls, was shot and killed by the police.

Claire explains what she knows about Ward X, indicating it's in the basement and offering to lead them there. She does this, of course, proving the truth behind everything, but when she finds herself in the corridor of cages, she finds that the detective lied-- Alice is still alive in her cage. The detective reveals that his daughter has leukemia and this may be his only chance to help her.

As Claire is brought past Malcolm's cage, seeing that he's been kept as a victim, she's strapped to a seat and told that she will be the next test subject in line. Finally, she can make the difference she always wanted to.
