Answer: Mercy Hospital

The fictitious Mercy Hospital was located in the Mission District of San Francisco. The patient was an elderly woman who was in for kidney dialysis. Dr McCoy muttered something about the Dark Ages and gave her a pill. A bit later on, they passed the same lady and a group of doctors, who were trying to figure out why she was growing a new kidney.

St. Swithins was the hospital in Richard Gordon's "Doctor" series of books. Royal Hope was the hospital where Martha Jones was when she met the Doctor. County General was featured in "E.R.".
2. What was Doctor McCoy doing in a 20th century hospital?

Answer: Rescuing Chekov

Chekov had suffered a head injury that caused tearing of the middle meningeal artery. By the time the others found him, he was in surgery, about to have holes drilled in his skull to relieve the pressure. Kirk and the rest of the team held the surgical team at phaser point while McCoy used 23rd century technology to repair the damage.

Uhura had already beamed out but there wasn't enough time to pick up Chekov before the Marines turned up. He was questioned but managed to escape. Unfortunately, on the way out, he slipped and fell.

Answer: To re-crystallise the dilithium crystals

The dilithium crystals had been completely drained and were breaking down. Spock theorised that high energy particles from a fission reactor could be used to reconstruct them.

Answer: Undergoing a slingshot manoeuvre around the sun at Warp 9

They performed this slingshot manoeuvre in order to return to the latter half of the 20th century. The resulting strain on the engines caused the dilithium crystals to decay.

Answer: A Klingon Bird of Prey warship, renamed the HMS Bounty

"The Voyage Home" took place just after the events in the "Search for Spock", during which the Enterprise was destroyed. The crew managed to escape the Genesis Planet in a Klingon ship and ended up on Vulcan. They were on the way back to Earth to face charges for the theft of the Enterprise in the earlier movie.

Kirk was given the command of the new Enterprise 1701-A at the end of the movie as well as being demoted to Captain.

Answer: They needed to retrieve some whales.

They had to go back in time to get some Humpback whales, since they were extinct in the 23rd century. A space probe was trying to communicate with the local cetacean life but, when it got no response, it started to adversely affect the weather, resulting in havoc all over the globe.
