Answer: The Gift

Kes, whose powers were becoming stronger, felt herself becoming a danger to the crew. When Seven tries to make contact with the Borg, Kes uses all her power to throw the ship clear of Borg space (well, the part of space that the Borg had completely taken over, at least) and disappears, never to be heard from again.

Or at least until "Fury."
2. The Doctor wanted to partake in this sport in the episode "Drive."

Answer: golf

B'Elanna was tyring to get together a weekend of holodeck time and the Doctor's time was the last piece she needed. However, the Doctor wanted to go golfing. He suggested a place that Tom and B'Elanna could go and the Doctor could use the same Holodeck and golf at the same time.
3. Last name of the guy who saved Woodstock. Well, according to Q, anyway.

"Death Wish" Maury Ginsberg operated the spotlights at Woodstock. Because of the help of Quinn (another Q), he noticed that something was unplugged which could have stopped the entire concert.
Side note: The actor who played Maury Ginsberg was named Maury Ginsberg. According to the "Star Trek Encyclopedia," it seems that once the producers cast the actor, they liked his name that they used that for the character.

When Samantha is trying to decide on a name for her child, it is suggested she use her husband's name. She ends up having a girl (thankfully?) and settles on Naomi.

She's blonde, curvy and her only line is "AAAAAAH!" At one point, Harry and Tom tried to get Seven to play the role. Seven ended up disabling the robot in 2 seconds, ruining the rest of the story for Harry and Tom.
