After the choir sings "We Wish You a Merry Christmas", Stan introduces the scene where Jesus is about to be born. Wendy is seen as Mary, while Kyle is Joseph and Kenny is an angel. As Mr. Garrison instructs Kyle to hold the baby from the legs and not the head, Mrs. Broflovski interrupts, arguing that their family doesn't celebrate Christmas and that Kyle shouldn't play Joseph because he's Jewish.

A group of religious people then say that The Nativity is the essence of Christmas, and getting rid of it would also imply getting rid of "Santa and all of that garbage".

Another group calls for the stop of tree-cutting for Christmas trees. Finally, Uncle Jimbo (Stan's uncle) stuns the crowd when he complains about the "little flaps on coffee lids", arguing that "if you don't want to spill your coffee, you shouldn't be driving with it." The crowd takes some time to process what they just heard, and then they agree!

Kyle is sent to his bedroom to think about what he did. In Kyle's bedroom, Mr. Hankey appears again, and suggests the boy to wear socks to sleep to avoid catching a cold. They agree that they will go together to school the next day to prove Mr. Hankey is real.

Mr. Hankey always greets whoever he's with using the words "Howdy Ho". He even mentions them twice in his song!

However, only Kyle sees that, since everyone else only sees Kyle throwing poo at Cartman. He is sent to the counselor, Mr. Mackey, who diagnoses Kyle with fecophilia. His friends then commit him to a mental hospital.

New York composer Philip Glass is invited to play his "minimalistic music" for the play. The crowd is disappointed because the play turns out to be very boring. Chef, who fittingly is the school chef, asks the children where Kyle is, and they answer that he is in a mental hospital because he couldn't stop talking about a Christmas poo. Chef asks if that's Mr. Hankey, and the children realize it is real. They decide to believe and Mr. Hankey appears, stopping people from fighting and focusing on the good parts of Christmas instead of the bad parts. Everyone goes to the mental hospital to get Kyle, they all sing the Mr. Hankey song, and they get their Christmas presents.

Interestingly, Kenny doesn't die in this episode as usual, and the kids indirectly acknowledge this, when they say at the end of the episode that "something feels unfinished".
