The episode begins with Lisa waking Bart up at 6:00am. The issue: Lisa is turning eight years old, a monumental age in her mind. She tells her brother that she's almost double-digits (a time, Bart says, which starts to take a toll on the body and the mind) and that he needs to get her a good gift to make up for the failed birthday attempts he's done for her in the past.

He agrees that he will, much to her delight. Of course, the tides start shifting soon after.

Over breakfast, Bart is asked to watch Maggie while Marge takes out the laundry. This turns out to be a short-lived chore; reading a box of Krusty-Os he opts to phone the expensive Krusty Hotline and Maggie, as she is wont to do, causes a bit of havoc by grabbing onto the cord for the ceiling fan and taking a ride around the kitchen. Marge comes in at that point.

Homer, looking for his clean, white shirts finds that Marge has washed them and they've all turned a bright pink. The problem was actually caused by Bart who, not thinking of the consequences, threw his red hat in for a wash. This, of course, leads to a strangling. Marge tells him not to worry either-- no one will notice.

Someone certainly does notice... in fact, everyone notices. During the morning trudge through the front doors, Mr. Burns picks up on Homer as a 'free-thinking anarchist' on his colour monitors and demands that something be done. Meanwhile, everyone else chatters about his attire. Lenny and Carl end up jokingly giving him a pink doughnut before Burns' lanks pick him up for an interrogation, x-rays, and body cavity search. Burns still demands to know the extent of Homer's insanity, so he sends him home with a Dr. Marvin Monroe personality test to be the deciding factor.

Homer, in an attempt to eschew the responsibility of having to fill out his psychological evaluation, tries to pass off the paper to Marge, who refuses to do it since it's an intimate look into his own thoughts. He goes to Lisa next, but Homer opts out of the deal when she tries to read him morbid poetry. Bart ends up being the final option, so the boy fills it out with his father's help. What follows, however, is Homer answering the questions almost impulsively, while distracted by other things (watching "America's Funniest Home Videos"). All of this turns out to be a horrible idea. Based on Bart's careless answers to Homer's tests, he's deemed psychologically unstable and sent to the New Bedlam Rest Home for the Emotionally Interesting.

Homer ends up subjected to Rorschach tests in an attempt to find the root of his issue, and while he ends up giving perfectly harmless answers to most of these, one instantly infuriates him, reminding him of Bart. The doctor, declaring him insane, stamps his hand with a red INSANE and places him in his room.

Despite Homer's apprehension, it turns out that his roommate is actually quite nice for a large, balding, white guy... of course he claims to be Michael Jackson, so there's that.

'Michael Jackson' insists that he's actually THE Michael, even though Homer really has no idea who that is. Despite Michael's attempts to run through a songlist and perform "Billie Jean" for Homer, none of it really helps. He does, however, sympathize with Homer because he seems like he's in the loony bin for no real reason (after all, Michael had some odd fashion trends himself).

Back at home, Bart speaks to Lisa about her birthday before receiving a call from the mental hospital; Michael calls on Homer's behalf and Bart writes down a message to hand off to Marge, a bit starstruck because of the conversation of course.

Fortunately, she's able to talk some sense into the doctors who, after discovering that Bart is a real person in Homer's life, realize that he's not insane. Homer is free to go.

While everyone is out, Lisa celebrates her birthday in a quiet kitchen-- only Maggie sits at the table with her. Considering the recent events, everyone has forgotten.

Homer, on his way out of the nuthouse, tells Michael to visit whenever he regathers his marbles and discovers that the singer is actually only there on a voluntary basis and is free to leave whenever he wants. Homer calls home about his release and the family's guest, and Bart, unable to hide the truth for more than a minute, calls Milhouse... and the word spreads instantly.

Michael takes a walk through the house as the family goes about their business. While Homer tries to scrub the stamp off his hand, Lisa tries to pen a letter in anger. Michael tries to convince Bart to write his sister a song for her birthday and, surprisingly, it works.

They spy on her from the treehouse and, eventually, get inspiration overnight. The next morning they push the piano up the stairs into her room and sing a 6:00am duet, in her honour. She's shocked at the gift and forgives her brother for everything. Immediately after the song, however, 'Michael' reveals to the Simpsons that he's actually Leon Kompowsky, a bricklayer from New Jersey who decided to start doing good for people.

He leaves the family shortly after.
