As "God", Castiel killed people doing bad things, like those spewing hate in God's name. He also cured leprosy, gave a blind man his sight, and forced the Ku Klux Klan to disband.

Fortunately, Bobby told Sam stabbing the God with a ram's horn would get rid of him for a couple centuries. Sam was able to stab Osiris with a ram's horn before the death sentence could be carried out, thereby saving Dean's life.
