Inconsistencies arose, and when Riker learned that his purported 'wife', Min, was the 'Minuet' from a holodeck program in the past, he knew that he was trapped in some kind of elaborate simulation. The character charading as Riker's 'son' turned out to be an alien creature alone on the planet, yearning for companionship.

If you checked Tom Paris you got the right actor but the wrong series (Robert Duncan McNeill played both parts). Joshua Albert was the cadet who was killed and Sito Jaxa was a member of the squadron in this season five episode. They were attempting the Kolvoord Starburst, a banned maneuver in which five craft fly within ten meters of each other and ignite their plasma trails, creating a star burst effect. Locarno talked the squadron into trying it to impress the people gathering for the graduation ceremony.

At a conference, Picard was ambushed by Lenarians who shot him with a compressed tetryon beam, causing his artificial heart to short-circuit. Q gave Picard the chance to change the event which required him to get the artificial heart (as a newly-graduated ensign, Picard was stabbed through the heart by a Naussican during a bar fight!). The episode follows Picard on another path in life, one which was boring and routine. At the end, Picard told Riker that, in trying to trim a few "loose threads" from his past, he unraveled the tapestry of his life.
