1. This episode is the only one in the series to feature the title of the episode after the opening credits. What episode number does it claim to be?

Answer: Twenty-three

For the first and only time in the run of the show, the episode featured the opening sequence and was followed by the credits playing in front of Bart's escapades through Springfield. This was then followed by the title, 'Episode 23: "Bart Gets Hit By A Car"'. It would be the only episode to do this (in hundreds) and, ironically, it would give a bit of the plot away.
The episode begins with Bart riding his skateboard around the town, through some wet cement and over occupied manholes. Generally, he acts as a menace. This, of course, comes to a head.
2. Who hits Bart with their car?

Answer: Mr. Burns

Riding his board into the road, Bart is immediately struck by Mr. Burns' expensive luxury car. Smithers checks out front to see that he's okay (while Burns suggests tossing him a nickel and departing) but they examine him to ensure he's not dead.
Bart has an out-of-body experience in the meantime, watching the clouds part and seeing a golden escalator ascend into the heavens. On this particular escalator he has the opportunity to see his great-grandfather on the Simpson side strangling a young child (a la Homer and Bart) and he also finds Snowball, the family's first cat.
3. What causes Bart to descend into Hell during his out-of-body experience?

Answer: He spits over a guardrail

When Bart boards the escalator he is asked to hold onto the handrails and avoid spitting over the sides, something he decides to do anyways. Because of this he plummets down into the depths of Hell, landing on a pillar and meeting the Devil. He's immediately told that he belongs in Hell for all of the evil deeds he's committed in his lifetime; the spitting over the ledge simply clinched it.

When Satan pulls up Bart's file he discovers that he's not due to arrive in Hell for at least another century, so Bart is sent back up through the Earth to his body, lying in a hospital bed.

When Bart awakens in the hospital he finds all of his family surrounding the bed along with a man named Lionel Hutz, who chased the ambulance all the way to the hospital and followed them in. He hands over his card (which turns into a sponge) and says he's willing to act as legal counsel for Bart to bring the man who hit him to justice. Homer says it isn't serious though-- Bart only had a bump on the head and a broken toe. Hutz says they should cash in instead but he's forced out by the family.

He invites them to his office at a later date.

At work, Mr. Burns calls Homer into his office to meet with his attorneys (Burns assumes they're meeting for the first time though it's clear that they've spoken on several occasions about serious matters). The attorneys suggest that Burns should pay Homer so he offers a $100 cheque.

When Homer notes that the hospital bills alone cost more, Burns retracts the offer and suggests taking him to court... so Homer heads off to visit Lionel Hutz where he neglects to notice a lack of posterity in the workplace. Hutz promises Homer a big cash settlement of a million dollars (at a 50% fee) and hands over a $99 strand of faux pearls before Homer likes what he's hearing.

After Homer decides to enlist the legal assistance of Lionel Hutz, he and Marge are asked to visit Dr. Nick Riviera, a doctor only a few doors down in the mall, for an opinion. Dr. Nick says that Bart is much worse off that expected-- the x-rays show whiplash and trauma and he may even have facial tissue damage (requiring excessive bandages). Bart, already in a neck brace and wheelchair, says that he's starting to feel better; he played baseball earlier in the day. Marge says she trusts Dr. Hibbert's diagnosis. Mr. Burns receives his court summons in regards to the million dollar case and plans to fire Homer but Smithers talks him out of it, saying it may be bad press.

Hutz urges Bart to lie on the witness stand and Lisa and Marge wonder why they need to stretch the truth so much, especially considering Bart is feeling better. When they finally arrive in court, Mr. Burns instantly paints a horrible picture of himself, claiming that he should be able to hit as many children as he wants.

When Bart hits the witness stand, he overembellishes the story to get the sympathy of the court and then Burns tries to do the same, but fails. His testimony that Bart intentionally struck his car doesn't go over too well and his lawyers suggest a settlement.

The Simpsons are invited to the Burns Manor to discuss a settlement (over a great deal of wine) and Burns offers a $500,000 settlement before leaving the room to leave Homer and Marge to discuss the proposition. Marge says that they might as well take the money to put it all behind them but Homer says they shouldn't; Burns is only offering the cash, half of the original amount, because he knows he'll lose in court, and backing down would not be worth the cost. Marge is shocked that Homer is deciding to be greedy, especially with the shifty lawyers and phony doctors involved. Burns retracts the offer and releases the hounds.

In court, Marge is called to the stand and, as per her character, decides to tell the truth. The opposition asks specifically about Dr. Hibbert and why they went to Dr. Nick Riviera instead-- she says that she didn't trust Dr. Nick and assumed he was a fake.

When asked to put a dollar amount on the lawsuit, she says that had Bart not been injured, he would've been paid $5 to take out the trash. After deliberations, Burns and his attorneys offer Homer and Hutz a sum of $0, an offer Hutz suggests they take.

At home, Homer is disappointed in Marge for losing the million and decides to go out for awhile.

At the bar, Moe says Homer's probably better off since rich people usually don't end up happier. Marge decides to follow him into the bar and asks him to forgive her for doing the right thing. Homer says that it's not about money. He's worried that he can't see her by his side anymore-- she blew his chance.

He's scared he might not love her anymore. Marge says he needs to look her in the eyes and find out if he does or not. When he finally meets her gaze, he can't resist. He loves her more than ever. For the next fifteen minutes, Moe, taking advantage of the situation, takes one third off the price of any domestic pitchers... no sharing.
