At the movie theater in Springfield, Grandpa Simpson and Jasper demand refunds for their viewing of McBain, a movie they felt very negatively about, and for their opinion of the theater which, as is usually the case with theaters, was quite disgusting. When the employee says that they don't allow refunds and that he shouldn't have a heart attack over it, Grandpa does, in fact, begin to have a heart attack.
The rest of the Simpsons, meanwhile, say a blessing before dinner. While the kids argue, Marge picks up the phone in the kitchen-- it's the hospital.

When the family heads to the hospital they find Grandpa hooked up to the life monitor. Dr. Hibbert says that he only had a 'minor' heart attack before the family leaves Homer with his father. This is when Grandpa takes the opportunity to tell his son that he actually has a half-brother.

When he and Mrs. Simpson were courting he slept with a woman who worked with a traveling carnival. Upon her return the next year she had a child; they left him at the Shelbyville Orphanage and never saw him again. Abe vowed to his wife that he'd never tell Homer the truth. Homer vows to find his half-brother no matter what it takes.

On their way, the family hops into the car and drives off to find Homer's half-brother. Their first stop is the Shelbyville Orphanage which has, since Grandpa's time, moved across the street. Although the director of the orphanage is unable to release the whereabouts of Homer's sibling, he does say that the boy's name was Herbert and that he was adopted decades earlier by a Mr. and Mrs. Powell.

The Simpsons pack their car and head off on a road trip. En route, the family is struck by odd occurrences; the police stop the car but let them go upon seeing Homer behind the wheel while another man seems to recognize Homer in the restroom.

After a heated business meeting at his motor company, Herb finds Homer and the Simpson family waiting outside the gate to his mansion. Herb, clearly a lonelier man, is thrilled with the family; he calls Homer the richest man he knows and Homer feels the same way about Herb. Homer seems more thrilled with the in-house cook and the ability to order pork chops whenever he wants more than anything else.

During a gala unveiling, Herb decides to introduce his half-brother's company-saving car, The Homer. After a brief commercial prelude, the vehicle is revealed and the stockholders' jaws are dropped. The monstrosity costs $82,000 and Herb's company is ruined; no one would ever buy the Homer.
Powell's motor company is purchased by the Japanese Kumatsu Motors and Uncle Herb loses his home and possessions. Herb, furious, boards a bus and says that as far as he's concerned he has no brother. As the family packs their own things, Grandpa arrives in a taxi to hear the bad news.
On the way home, Bart tells Homer that he thought the car was actually really cool.
